0914335601 2013-10-22 Preliminary Restoration No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 2 100-barrel steel production tanks and 1 50-barrel brine tank, and separator unit. The wellhead and battery are both enclosed in separate chain-link fences. The load-outs are bull-plugged. The ID and secondary containment are good. The unit is fitted with carbon canister, low-pressure relief valve, and spark arrestor. Gates are locked. Lease road is in good condition and areas impacted by drilling have been restored. No problems noted and passes preliminary restoration.
1203960073 2009-03-24 Drill / Deepen / Reopen No Violations Columbus received a call from Representative Mendells offive on behalf of Dr. Michael Frankel. The complaint was forwarded to me on this date and concerned 1) noise, 2) fugitive light, 3) stockpile locations and 4) the proposed location of the tank battery. I called and left a message at Dr. Frankels home and office on 3/24 before speaking with him on 3/25. I explained what activity has yet to occur and when it would happen. The noise and light issues were resolved when the drilling rig moved off on 3/23. The soil stockpile location was appropriate and in the areas defined in the pre-permit site inspection. The location of the tank battery was between Ohio Valley and the land owner. The location that was proposed meets the legal requirements, which as I explained was what I could regulate. He indicated that he would speak to Moss and request that the location be changed either to the north or west.

1304214719 2014-08-26 Plug / Plug Back No Violations I was onsite for preliminary work being conducted to plug and abandon the well. Crews from J-Well are on site dismantling surface equipment and removing 1.5-inch production tubing. Cementing crews from Petroset will be onsite on 08/28 to begin setting plugs. No problems or issues of concern during this visit.
1382577824 2009-03-19 Drill / Deepen / Reopen No Violations Wildcat is drilling ahead and should finish within the next 36 hours. The well is being drilled with fluid as required by permit. The pits are lined and in good condition. The entire location is fenced with either chain link or snow fence. A noise and light complaint from the neighbor southeast of the well (Dr. Michael Frankel) was received in Columbus some time earlier in the week. The complaint was not relayed to the field until a week later.
1409940585 2014-08-28 Plug / Plug Back No Violations I performed an on-site inspection during operations to plug and abandon the well. Crews from J-Well are on-site to perform plugging activities. Fencing enclosures at the wellhead and battery have been removed. The well is being blown down ahead of cementers arrival (Petroset). The bottom (formation) plug was bullheaded through the 4.5-inch casing. While setting the bottom plug, it was observed that the well began to off-load gas and fluid. After several attempts to pressure up and shut it the plug, the gassing continued with 1 to 2 barrels of fluid return. I advised that gas-cut cement was a possible cause. I recommended that the pressure be allowed to bleed off until static conditions were established and then a second formation plug set through 1.5-inch tubing directly on top of the previous plug. !.5-nch tubing was run in and an additional 20-sack plug set and the well was shut-in overnight.

The former wellhead area has been graded and re-seeded. The former tank battery is being restored at the time of this inspection. Approval of final restoration is pending this final work.

1447695651 2015-11-16 Final Restoration No Violations I inspected the former location of this producing unit as an evaluation for the completion of tasks necessary for final restoration. The former locations of the well, well pad, and tank battery have been restored. Access to the well and battery which were afforded by a shared use asphalt drive and a short gravel drive, which remain per landowner agreement. The flow lines and sales line were removed per report of Ohio Valley Energy. No further action is needed and Final Restoration passes.

The former area of the wellhead has been restored. The former battery location has been re-graded and is awaiting re-seeding. All production appurtenances have been removed. The short access road to the battery remains gravel -paved and will require removal. The requirements for final restoration have not yet been met.
1949846410 2014-08-29 Plug / Plug Back No Violations I was onsite for completion of on-going plugging and abandonment activity. The bottom formation plug was set on 08/28/2014 and shut-in overnight. Upon arrival this morning the well was quiet with no residual pressure. The 4.5-inch casing was ripped at 2,440 feet and the balance of the casing removed. A standard 300-foot formation plug was set at the Big Lime formation, and a 2-stage surface casing plug was set from 400 feet below grade to surface, both through the 4.5-inch casing during removal. Circulation fluids were collected in a 100-barrel half-round steel tank. The remaining activities including placement of steel lid with API#, and final restoration work are still pending.
