At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery in separate enclosures, both located within the asphalt-paved parking area of a multi-tenant business center. The enclosure gates were locked and secure. The load-line valves were secure. I had noted previously that the production data was missing, but subsequently found that the data was being reported under this API number (34-035-2-2077-01) and not API Number 34-035-2-2077.

At the time of this inspection I observed the following: a wellhead with plunger lift system and associated tank battery in separate enclosures which were locked and secure. The valves and connections at both the well and battery were secure and free from signs of leakage. Process safety equipment was present. ID signage was present and accurate. The areas surrounding the well and battery were comprised of asphalt parking and drive areas for the hotel facility located on the parcel. I walked the surrounding areas and found no signs of impacts or degradation. Access to the well and battery was afforded by shared-use, asphalt-paved driveway which was in good condition.

Operator has a reinforced liner on site for the pit. Permit sign is posted. Permit is on site. Wire fence is up around site. Safety signs are up. Well is 101 from the building. Bottom and sides of the pit were shale and cuttings, no water noted.

At the time of this inspection, there is 12 to 14 inches of snow cover which obscures some surficial features.

Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. There is has been no production reports filed since this well was completed in 2010. I contacted Larry Gibler of Duck Creek regarding the missing data. I also contacted Vicky McKinney (ODNR-Columbus) and determined that the missing production data had been misdirected. The data is being moved.
