The production wellhead is fitted with a pumpjack system and is currently producing. The pumpjack unit is enclosed in an 8-foot chainlink enclosure which is locked. The tank battery consists of 3 100-barrel steel tanks, and separator unit. The battery is enclosed in a chainlink enclosure. The secondary containment is present and in good condition. The tank battery is shared with the Sokol 1 Unit. The units are accessed through a locked gate, which does not have an ID. I called Tony Carlyle with Equity Oil and asked him to put an iD sign at the gate. He indicated that he would take care of it.

0810032534 2015-05-22 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well is comprised of a pump jack in a chain-link enclsoure which is accessed via a shared access road. Fittings and conections at the wellhead are secure. There is some oil staining around the wellhead which requires clean-up. At time of this inspection, I observed the following deficiencies: two of the four production tanks do not have hatch lids capable of sealing, load lines are not protected with locks, ID signage is present but lacking required information, locks are required on electrical panels. In addition, there is oil staining inside the battery near the load-outs and inside the wellhead at the back-pressure-valve which requires clean-up. I have notified Equity Oil & Gas that these deficiencies must be corrected as soon as practicable or by 06/29/2015. 0867471620 2015-04-02 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well is located within the camp facility, and the tank battery is located near the front entrance to the camp facility at the referenced address. The well is comprised of a pump jack system in a chain-link enclosure. The battery is comprised of 3 100-barrel steel production tanks and mechanical separator. At the time of my inspection, I noted some oily sheen on water draining from the wellhead area. I contacted Tony Carlyle with Equity and advised him about the oil. I recommended that Equity construct a containment berm at the wellhead owing to the drainage gradient at the site. Tony indicated that he would have crews at the site within 24 hours to clean up the sheen. Otherwise, fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. The access road is good, passable condition with no washouts or other erosional deficits or signs of impact. The battery is shared with the #2-1845 well located near the tank battery within the facility.

Production reports were current and up-to-date. The shared-use access road was in good condition. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation.

At the time of this inspection, I observed the following: a wellhead with pump jack system in a small, chain-link enclosure. The wellhead fittings and connections were secure and free from problems or leakage. I noted that the ID signage had become detached from the fence and I notified Tony Carlyle with Equity Oil. The tank battery, which is shared with the # 2-1845 well, is located near the entrance to the private recreational facility. Fittings and connections at the battery were secure. Load-line valves were locked. The enclosure gate was locked. Tank lids were closed and sealed. I observed no other problems or deficiencies.

1948330034 2011-09-24 Urban Drill / Deepen / Reopen Violations Noted On 9/24/2011 Inspector Norburt Lowder received a call from Wade Balser of OEPA regarding a complaint he received from the Broadview Heights Fire Department of oil bubbling up in the back yard of 8490 Wyatt Road in Broadview Heights, Ohio. This property is owned by Irene Krotz and is located immediately south of the Kral 1D (#2187). Mr. Lowder then contacted Gene Chini and Ray Wallace of ODNR, Gonzoil and Mrs. Krotz about the problem. DNR Inspector Wallace arrived at the site at approximately 5:15 PM on 9/24/2011. He inspected the back yards southeast of the well site, ( Krotz, Tremblay and Benes), and discovered evidence of petroleum on the grass in a small depression that ran through the Krotz and Tremblay back yards. He then investigated the Kral 1D drilling site and noticed the soil adjacent to the drilling rig air compressor and mud pumps was covered with PeatZorb. There were small of motor oil on the ground surface which apppeared to be flowing into a small pool of water that had collected on the east side of the location. DNR Inspector Wallace also discovered some broken clay field tile adjacent to this pool of water and, upon further investigation, thew opening to the solid clay feeder tilewhich appeared to run south along the eastedge of the excavated portion of the drill site. It appears that the motor oil had made its way to the pool of water and into the drain tile. Excavated two (2) holes by hand to locate the field tile outside of the drilling location. OEPA had straw bales placed around the second (farthest) excavated area and approved that work be terminated until Sunday, 9/25/11, due todarkness. On Sunday four (4) additional holes were excavated to locate tile. Eventually found the end of the field tile in what apppeared to be a french drain located on the property line between Benes and Tremblay. There was a second drain tile that emptied into this french drain from the east. The first drain tile was flushed with fresh water and all water/oil colle
