0134987034 2014-07-16 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 05/18/2009. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located down a dedicated access raod with entry off of Snowville Rd.. The access road is gravelled and in reasonable condition. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located along the the access road, near the main road. The battery is comprised of a total of 2 steel 140-barrel production tanks, 1 50-barrel poly brine tank, and horizontal separator. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition.
0213888571 2018-11-14 UrbanProduction Wells No Violations Moore Well Service was on location pulling rods in order to replace the downhole pump. The steaming pit was lined with a plastic liner. The well was operated as a gas powered pumping well. The tank battery was surrounded with a locked wooden fence. Legible I. D. signage was posted on the fence. The load lines were locked and plugged. The tank battery consisted of two (2) 140 barrel stock tanks, one (1) 75 barrel poly brine tank, one (1) vertical separator and one (1) gas measurement meter. The 140 barrel tanks were compliant with the required equipment per HB 278. No leaks were identified.

1370551511 2013-06-06 UrbanPreliminary Restoration No Violations The unit consists of a production wellhead with a pump jack and a storage tank battery. The wellhead and battery are enclosed in separate board-on-board enclosures, which are locked. Unit ID is present. The tank battery consists of 2 140-barrel tanks and 1 25 barrel tank, and a mechanical separator. HB 278 equipment is present. I did not observe any problems or violations, This unit passes preliminary restoration.
2031126806 2017-03-16 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery located in a clearing within an otherwise wooded area. The wellhead and battery were in separate board-on-board enclosures which were locked. Access to the battery and wellhead was afforded by a gravel-paved access road which was snow-covered, but accessible. The battery was comprised of two 140-barrel steel production tanks, poly brine tank, and horizontal separator. The fittings and connections at the wellhead and battery were secure and free from signs of leakage. The load-lines were plugged, locked and secure. Secondary containment was present and intact with no signs of lekage, spills, or problems. The ID signage was present and accurate. I walked the areas surrounding the wellhead and battery and found no signs of impact or degradation form the on-going production.
