At the time of this inspection, I observed the following: Ingress and egress to and from the well area is afforded by the shared house driveway with short, unpaved section that extends north to the wellhead and battery. A wellhead with mechanical plunger lift system is located in a board-on-board enclosure in an opening within an otherwise wooded back yard area. The well enclosure is missing one of the sidewalls. Fittings and connections at the wellhead secure and free from leakage. The 8.625-inch casing annulus was exposed and plumbed into the system. Areas around the wellhead consist of turf grass and woods which were free from signs of stress or impact. The tank battery is located 50 feet west of the wellhead within the same grassy and wooded yard area. The battery is comprised of one 100-barrel, steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids are closed but the production tank lid is an older style flap-style lid which is not compliant with. The load-outs were plugged and locked. The battery and wellhead are located within a densely wooded area and not otherwise screened. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issues: the production tank lid must be replaced or upgraded to meet the sealing requirements of OAC 1501:9-9-05(E)(4)(d), the wellhead enclosure is partial broken down and needs repair. I notified the owner about the maintenance defficiencies. |

At the time of the issuance of the compliance notice, there had not been production reported for three consecutive years, and the owner was directed to plug and abandon the well . The wellhead enclosure fence was broken down, the interior of the battery had become overgrown with vegetation. Flow-line valves at the wellhead were closed. Since the compliance notice, the owner has submitted a production report for 2016 that indicates 216 MMC of gas production but no other indications that the well has been placed back into production. |

During a prior inspection, I noted that the production tank lid was not capable of sealing and needed upgraded, and the well enclosure was partially missing. At the time of this inspection, the well was comprised of a plunger lift and tank battery system with one 100-barrel steel production tank and 50-barrel poly tank. The well and battery were in separate board-on-board enclosures. The previously noted deficiencies had not been addressed. The well was shut-in with no production reported since 2012. The well owner/operator shall submit a written plugging schedule to Inspector Stephen Tompkins at ODNR Uniontown Field Office, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Contact Inspector Tompkins at 330-907-4647 with any questions regarding this Compliance Notice. |
