Location of the well on 3 D Topo and the GPS reading I took, matched within .0002 N lat and matched long. Measured well with a tape from the corner of the house, distance was 170.6 feet, which is a 6 inch difference from Plat. Pits are under construction, 2 feet of sandy clay, remainder of the pit was broken sandstone and shale. Operator is going to clean rocks out of pit, pad with clay as needed. Reminded driller to make sure they have slack in the liner, to prevent cuts. Lease road is stable constructed per the permit. Operator was trenching in silt fence east side of the permit. Well spudded during inspection. Permit was on site in the dog house.

1361328841 2013-06-25 UrbanProduction Wells No Violations The producing unit is located in a residential area in Gates Mills. The above-ground storage tank (AST) battery is located at the end of an access road, approximately 480 feet SSW of Hillcreek Lane. At the time of my inspection, there is a cable gate at the access road entrance which is locked with a private padlock. The access road parallels the approximate western boundary of the house at 1190 Hill Creek Lane. The access road is comprised of dirt with some gravel within the initial 200 feet. The tank battery is comprised of 2 110-barrel steel ASTs, 1 50-barrel poly AST, and mechanical separator. The battery is enclosed in a 8 board-on-board enclosure. An earthen secondary containment dike is present within the battery, and is good condition. The system includes HB 278 equipment. I observed an area of heavy oil staining and ponded water with free-phase oil at the southeast corner of the battery. The area is approximately 150 square feet in areal extent. The oil appears to be confined to the interior of the dike. I did not observe any oil or staining outside the dike. I did not observe any active leaking from the system . The chart recorder indicates the well has not produced since May 2013. The producing wellhead is located 175 feet to the east, and is comprised of a wellhead assembly with mechanical plunger lift system in a 6 board-on-board enclosure. There is also a domestic meter at the wellhead. I did not observe any problems or violations at the wellhead. Unit Identification is present at both the wellhead and the tank battery. I spoke with Jon Visner of Seagull Development Corp. He indicated that the Newtons have denied access to the well system since May 2013, and have place the cable gate at the access road entrance. I also spoke with the homeowner (Mrs. Tina Newton) who also indicated that they are engaged in a dispute with Seagull.
1372724864 2013-06-28 UrbanProduction Wells Violations Noted The producing unit is located in a primarily residential area. The above-ground storage tank (AST) battery is located approximately 480 feet SSW of Hillcreek Lane. The access road to the tank battery is not constructed out of durable material and is not constructed in a manner sufficient for the ingress and egress of fire and emergency response. The access road needs to be brought into compliance with OAC 1501:9-1-02(E). The tank battery is comprised of 2 110-barrel steel ASTs, 1 50-barrel poly AST, and a mechanical separator. The battery is enclosed in a 8 board-on-board enclosure. Portions at the rear of the enclosure have a significant gap between the bottom of the fence and the ground. An earthen secondary containment dike is present within the battery, and is in good condition. Safety equipment is in accordance with 1501:9-9-05(E) . I observed an area of oil staining and ponded water with free-phase oil at the southeast corner of the battery. The oil appears to be confined to the interior of the dike. I did not observe any oil or staining outside the dike. I did not observe any active leaking from the system . The producing wellhead is located 175 feet to the east, and is comprised of a wellhead assembly with mechanical plunger lift system in a 6 board-on-board enclosure. There is also a domestic meter at the wellhead. I did not observe any problems or violations at the wellhead. In accordance with the permit, the access road for the wellhead is comprised of a shared portion of the residential driveway at 1190 Hillcreek Lane. Beyond the driveway, the access road has not been established. ODNR requires that either the permitted access road which utilizes the residential driveway or the access road along the western property boundary be established in accordance with OAC 1501:9-1-02(E). The oily staining inside the AST battery should be remedied and the wooden enclosure repaired. I advised Seagull Development Corp. of the findings and ODNR requires the items
1378728958 2013-09-06 UrbanProduction Wells No Violations I inspected the unit pursuant to a call from the landowner regarding noise from the pumping unit. The landowner stated that the pumpjack which has recently been placed at the wellhead is noisy. The pumpjack unit is gas-driven and appears to be functioning normally. The noise level is within the normal range for a gas-driven pumping unit. The landowner indicated that the unit has been running continually during the daytime hours and cycles off at night. I advised them that I would contact Seagull and detail the complaint. I spoke with John Vizner of Seagull and discussed the noise issue. He indicated that he would follow-up with the landowner and discuss possible alternatives.

1474053153 2016-09-16 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: the former pump jack system was dismantled and removed leaving the wellhead with production tubing and pump rods in place. Flow-line valves were shut. The board-on-board enclosure which had previously housed the pumping unit had been dismantled and removed. The tank battery comprised of two 100-barrel steel production tanks, one plastic 50-barrel brine tank, mechanical separator, and associated production appurtenances. The board-on-board enclosure formerly located at the battery had been dismantled and removed. A review of reported production indicates the well has not produced in saleable quantities since 2012. Based on the status of the well at the time of this inspection, the owner must do the following: The well owner/operator shall submit a written plugging schedule to Inspector Stephen Tompkins at the Uniontown Field Office at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1692210305 2016-02-04 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: Ingress and egress to and from the well area is currently afforded by the shared house driveway. The former access path used for battery access has been blocked and gated by the current land owner in a dispute with Seagull Development Corp. A wellhead with pump jack system is located in a board-on-board enclosure in the otherwise open portion of the back yard. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed and plumbed into the system. Areas around the wellhead consist of turf grass which was free from signs of stress or impact. The tank battery is located 125 feet west of the wellhead within the adjacent woods. The battery is comprised of two 100-barrel, steel production tanks, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact with some minor amounts of standing water which was free from sheens or other signs of leakage or impact. Tank lids were closed. The load-outs are located remotely from the battery near the road, and are plugged, but one valve needs a lock. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issue: the production tank lid is missing the closure wing-nut. This needs replaced to effect a proper seal. I observed some discarded plastic gas cans and other debris which needs to be removed. Load-out valves require locks. I notified the owner about the need for closure wing-nuts, and locks for the loa
