Areas around the well and battery consist of a moderated wooded area with meadow grass. Much of the immediate surrounding area is being utilized by others for storage of building materials. I did not observe any signs of problems at the time of this inspection. I contacted the emergency number listed to confirm accuracy. The number is functioning and correct. |

The access road to the well is a shared driveway to a nearby residence and is asphalt to approximately 40 feet from the wellhead. No gate is in place and entry GPS is at the road entering the site. The well head is enclosed by board on board fence, locked and secured and the area around the wellhead shows no problems. The tank battery is enclosed by board on board fencing and has two 140 BBL storage tanks and one 50 BBL brine tank. Tank caps are closed and secured and vent caps and a charcoal filter are in place. There are load lines secured with locks and bull plugs with a metering run and one vertical separator unit in the tank battery. Urban Preliminary Restoration is complete and passed. |

Supervisor Stephen Ochs as a follow-up to a complaint reported by the City of North Royalton. The complaint was regarding the presence of construction materials being stored by the land owner, in various piles at the perimeter of the well location, including along the entry drive to the well. At the time of this inspection, we found the material to be comprised of residential construction elements, parts of construction machinery, and cut timber. None of the observed material was within the well or tank battery footprints, or the limits of the access road or well pad parking or turn-around areas. Access to the wellhead and battery was not obstructed. The wellhead pump jack system was in operation. The fittings and connections were secure. Secondary containment at the battery was intact and dry. We walked the areas around the well and battery and found the natural greenscape to be free from signs of impact associated with the on-going well operation. Production reports were current and there were no other problems or issues noted. |

At the time off this inspection, I observed the following deficiencies: the load lines and electrical boxes are not lock protected, the tanks are fitted with improper lids, the tanks require spark arrestors. These deficiencies must be corrected as soon as practicable or by 07/06/2015. In addition, I observed the following maintenance issues with this well . The secondary containment berm at the well and battery is eroded leaving a significant gap between the fence and ground surface. Also, the ID signage is present but requires additional information to be complete. Refer to OAC 1501:9-9--e) for ID requirements. Fittings and connections at the wellhead were secure. The pump-jack supports were solid. The stuffing box fitting was secure with no leaking or problems. I noted a small bit of oil floating on water ponded around the wellhead and I notified CJ Cutter about the need for clean-up here. The bridle attachments were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment had several inches of standing water from recent rainfall. The water was clear and free from any signs of spills, leaks, or other problems. Access to the well and associated battery is from a shared-use, paved access road that enters from Nicole Place I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity. The area immediately surrounding the well and battery has been used for materials storage by others. |
