The well and associated batery are located at the back of a church property. Elements of the production system had been vandalized previously. At the time of this inspection, the damaged system elements had been repaired and the system returned to production. Fittings and connections at the wellhead and battery were secure and free from signs of leakage. The secondary containment was intact.

At the time of this inspection, I observed the following: a wellhead with pump jack system and associated battery, in separate chain-link enclosure. The enclosure gates were locked. The load-line valves were plugged and secure. The tank lids were closed and sealed. The secondary containment was intact and dry. I walked the areas surrounding the well and battery and found no signs of problems or impacts.

The well and battery are located in a clearing in a wooded area behind the church. Access to the well is afforded by a gravel-paved access road which enters from W. Mill Rd. and proceeds west to the house and wellhead. At the time of this inspeciton, I observed the following: enclosure gates are locked and secure. Tank hatch lids are closed and sealed. Load lines are lock-protected and secure. Fittings and connections at the wellhead and battery are secure. Process safety equipment is present. The secondary containment is intact and free of standing water or staining. At the time oof this inspection I observed some oily staining around the base of the wellhead which neads cleaned up and the impacted gravel material replaced. This The shared entry road is well-drained and free from erosional deficits. The ID signage is not compliant with OAC 1501:9-9-05(E)(1)(a-e). I notified the well owner regarding the oil spill and ID signage.

The wellhead and tank battery are located behind the church located at the referenced address. The wellhead pump jack and battery enclosure gates are secure and locked. Load-outs are plugged and secure. The containment interior is dry. Connections and fittings are secure with the following exception: At the time of this inspection, I observed some oil staining around the wellhead from stuffing box seal leakage. I contacted Bob Ruefner, contract well tender for Moore Resources. He indicated that he would tend to the staining and replace the stuffing box seals within the next couple of days.

1493732604 2017-04-27 UrbanProduction Wells No Violations I inspected this well location pursuant to a call from Jeff Moore of Moore Resources. Jeff reported that the well tender had discovered that vandals had entered the wellhead and battery enclosures overnight and caused damage and spray-painted graffiti on the storage tanks. Upon my inspection, I found the wellhead and tank battery enclosures locked and secure. Jeff had reported that the tank valves had been opened, but the lines were isolated by the load-line valves so no fluid had escaped. The computer pump control panel had been damaged and the electrical panel lock was broken and fuses Various profanity was spray-painted on the steel storage tanks. None of the vandalism resulted in a release of petroleum fluids or gas. Crews from Moore Resources had already commenced repairs. Jeff Moore indicated that a police report had been filed with the City of Broadview Heights. There was no other damage evident to any structures or natural features outside of the production elements.

At the time of this inspection, I observed the following: a wellhead with pump jack system and tank battery in separate chain-link enclosures which were locked and secure. Fittings and connections at the wellhead were secure. There was a pressure relief fitting on the surface casing which was intact. Fittings and connections at the tank battery were secure. Process safety equipment was present and intact. Secondary containment was intact. Tank lids were closed and secure. I observed some oil leakage from the cross-over line at the tank battery and I notified Jeff Moore. I walked the area surrounding the wellhead and battery and did not observe any signs of problems.
