At the time of this inspection, I observed the following: a wellhead with pump jack system and tank battery in separate chain-link enclosures which were locked and secure. Fittings and connections at the wellhead were secure. The surface casing nipple was open to atmosphere at the surface and I observed no gas being emitted. Fittings and connections at the tank battery were secure. Process safety equipment was present and intact. Secondary containment was intact. Tank lids were closed and secure. I walked the area surrounding the wellhead and battery and did not observe any signs of problems.
1299044665 2015-06-19 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and battery are located at the rear portion of the Broadview Heights Fire Station on Wallings Road. Access to the well is afforded by a dedicated gravel-paved access road which continues north from the existing back parking area for the fire station. At the time of this inspection, I observed the following: enclosure gates are locked and secure. Tank hatch lids are closed and sealed. Load lines are lock-protected and secure. Fittings and connections at the wellhead and battery are secure and free from signs of leakage. Process safety equipment is present. The secondary containment is intact. The shared entry road is well-drained and free from erosional deficits. The ID signage is not compliant with OAC 1501:9-9-05(E)(1)(a-e). I notified the well owner regarding the ID signage. The owner has reported that new signs are being prepared.

1354206912 2012-11-29 UrbanPreliminary Restoration No Violations My visit to the well site today was to perform an Urban Preliminary Restoration inspection on the well. The well is located behind the Broadview Heights, Ohio Fire Station #1. The wellhead is enclosed in chain link fence which is locked and secure. The wellhead area is neat and clean and no violations were noted. The tank battery is enclosed in chain link fencing and is clear of debris and oil. There are two 100 BBL storage tanks and the hatches are closed and secure. There are vent units in place along with a charcoal filter system. There is one vertical separator unit in place. Load lines are locked and secured with bull head plugs. Well was reworked for pump replacement in May 2012. Urban Preliminary Restoration passed.

The well and battery are located behind the Broadview Heights Fire State, and accessed via a shared-use road. The access road was in good condition. Fittings and connections at the wellhead pump jack system and battery were secure and free from leakage or problems. The well was in production with an active chart record. I walked the areas surrounding the well and found no signs of problems or impacts relating to the production operation.

I inspected both the interior areas and exterior portions of the well and battery and surrounding areas and found no signs of impact or problem. I noted that the ID signage had become faded and difficult to read, and I notified Moore Resources.
