0027087392 2017-10-11 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located in the back of a residential property. At the time of this inspection, I observed the following: The enclosure gates were locked. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and observed an area adjacent to the wellhead exhibiting linear heaving and stressed vegetation. The condition had been noted during a previous inspection and discussed with the well owner. The cause of the impact is suspected to be related to the former pit location. I spoke with Mark Smith with PEP and he agreed to go evaluate and develop a plan for restoration.
0126845872 2018-08-07 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. I found the wellhead pump jack and tank battery connections to be secure and free from leakage or problems. The load-lines were locked and secure. Enclosure gates were locked and secure. Production reports were current. The well was in production with an active chart record. I walked and inspected the areas surrounding the well and battery and found no signs of problems. The well was in ownership transition and the new signage had been put up. The well was in active production. I noted that that the heaving at the former pits which I had noted and discussed with Mark Smith remained unaddressed. I contacted Mark again, and he advised that he would go inspect the site before the end of the week and then contact me with a plan.

0459653249 2014-08-19 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 09/15/2006. The unit is producing and production reports are current. The well is located in a upscale suburban area with large lots, and densely wooded areas around the well and battery. The producing unit is accessed via a dedicated, grass-covered access road which extends north from Dorchester Road. The access road is level, and well drained with no signs of erosional deficits. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery in separate board-on-board enclosures. The valve fittings, connections, and other production appurtenances appear to be well maintained and are free from signs of leakage or other problems. The battery is comprised of 1 steel 140-barrel production tank, 1 140-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. There is secondary containment at the battery and the interior areas are dry. I did not observe any signs or evidence of leakage, spills, or problems. Unit identification placards are present at both the battery and wellhead and are in good condition. I observed no issues or problems requiring attention at the time of my inspection.

1277752409 2010-06-20 Production Wells Violations Noted Received phone call around 9:30-10:00 am from Don Nething with Ohio Valley Energy on behalf of Fire Chief Robinson who had called Mr. Nething. He had found oil in the stream that crossed the teardrop area between Gates Mills Blvd streets. Don Nething confirmed that the oil was crude oil and not fuel oil. They then started to check wells to the south of stream .The lease names are Gormen,G.M.V.,Sopko and Teardrop. These Wells had no leaks. The next well in line was Giordano #1 P#1990. They checked around the dike and found a spot where oil was leaching out and went into drainage ditch.

I Arrived on site met Fire Chief Robinson , Reginald Brown OEPA and Harry Himelright (Well pumper for Pipes Pursie E) I will oversee the clean-up of tank Battery area and Reginald Brown OEPA to oversee the cleanup in the drainage ditch and stream . I asked Mr. Himelright how much oil was in the storage tank ,He said that on Friday he had just transferred the brine to the brine tank and he gauged the tank and it had 2.5 (apporximatley1000 gallons) of oil in tank. The tanks are the low clearance tanks (70BBLS).The cleanup started in creek by Inland Waters of Ohio and Scott Miller Contracting. Oil inside dike was dipped out and put in brine storage tank. Oil field water truck on site to vacuum out brine from a sump hole dug in gravel of the dike. Cleanup to continue on Monday.

1277757108 2010-06-21 Production Wells No Violations Met Rick Simmers on site. Scott Millers Crew was working on removing the storage tanks from dike area. Started to remove the gravel and found that the dike area had a clay base. In the northwest corner of the dike where the sales line ran through the dike, there was a drain (4diam) which lead to the outside of the dike near the drainage ditch. The 4Line was located at the bottom of the spill containment system and was covered with gravel. the line was open on the inside the containment system. A bullplug was loosely attached to the end of the line outside of the spill containment system. The crude oil leaked around the loose bullplug, ran down the bank of the drainage ditch and entered the drainage system. Remedial action included removal of the drain line repack clay around the sales line, and also add more clay to the tank battery area gravel was then spread to reset the tanks to reconstruct the tank battery. All affected soil and stone were transported to an OEPA approved Landfill for final disposal.

 While on site I checked clean-up in the ditch just north of tank battery that was impacted on 20th. No visible sign of oil on water or along the edge of ditch. Reginald Brown OEPA is overseeing the clean-up of the buried storm water drains and stream.

1278937094 2010-07-01 Production Wells No Violations On June 28th Mark Smith and Regie Brown OEPA phone conference to discuss further clean-up action needed at the culvert just north of containment system. Oil (approximately 2-3 cups) still leeching out of bank and into ditch. Then on 29th Mr. Smith and Scott Miller started to dig out the bank of an area about 3x4 and found the oil was coming out of a layer of fill material at approximately 4 below grade. Contaminated soil removed and oil absorbent pads were placed.

Mark Smith and I met on site and went over what was discussed on the 28th and 29th. Then excavated just inside containment system down to approximately 4 found same layer of fill material and removed contaminated soil and replaced with fresh soil

Locks are present and secure at the gate, enclosure entry points, and load-lines. The secondary containment is intact and free from standing water or signs of leakage. Valves and fittings are secure. Tank lids are closed and secure. Fittings and connections at the wellhead are secure and free from signs of leakage or problems. The wellhead enclosure area is well maintained. During my inspection, I observed the following: areas outside the enclosures of dead grass which correspond with former pit locations. I spoke with Mr. Giordano and he reported that these areas have not been completely restored since the time of drilling. I notified Mark Smith with PEP Drilling about the area. He indicated that he would evaluate. Any restoration worked required will likely have to wait until spring.
1497836075 2010-07-07 Production Wells No Violations Jay Cheslock and Myself met with Mark Smith (company) and Wade Pole contractor). To discuss the clean-up actions taken on 1st of July. A small sump will be placed approximately 2 outside the containment system to monitor the layer of fill.
1885017003 2013-07-24 UrbanPreliminary Restoration No Violations The producing unit consists of a wellhead with mechanical plunger-lift unit and above-ground storage tank (AST) battery, The wellhead is located in a board-on-board enclosure which is locked. The well is currently producing. There is a unit ID at the wellhead. I did not observe any signs of leakage or problems. The AST battery is comprised of two 140-barrel production tanks and mechanical separator in a board-on-board enclosure. There is a unit ID at the wellhead. Secondary containment is good. Screening is established around the wellhead and tank battery. Preliminary restoration passes.
