1327685858 2012-01-26 UrbanProduction Wells No Violations Mike Weinsz of M&M Royalty (well owner), contacted DNR Inspector Norburt Lowder on 1/25/2012 in reference a complaint from a resident living in the vicinity of this well. The resident stated that, since December 2011, there have been several tremors in the vicinity of her home and that these tremors move pictures on the wall and cause items to fall off of end tables in her home. She further stated that that several neighbors have experienced these tremors also. DNR Inspector Lowder referred this complaint to me, (Cuyahoga County DNR Inspector Ray Wallace). I met with Bob Allen, maintenance supervisor for M&M Royalty, at this well on Thursday, 01/26/2012, at 8:30 AM. We performed a status check inspection on this well which was operating at the time. No violations were found. At approximately 9:00 AM the well shut down. This well is set to run from 6:45 AM to 9:00 AM, Monday through Saturday. There was 240 pounds of pressure on both the 4.5 casing and the tubing. Shortly after 9:00 AM, I received a call from Mary Boehnlein, 6206 Timberlane Drive, Independence, Ohio who stated that she had experienced two tremors in the last five minutes. I assured her that neither Bob Allen or myself had experienced any tremors and that I didnt believe the well was the cause of the issue. I then contacted Paul Greco, Cuyahoga County ODNR Industrial Minerals Inspector, and asked him to help me in investigating this complaint. On Monday evening, January 30, 2012, I received a copy of an e-mail DNR Inspector Grecko sent to DNR Inspector Mike Mann, and several others, regarding this issue. A copy is in my complaint folder. This well is in compliance with 1501:09-5-5. No violations found. Complaint #3549 can be closed at the request of Inspector Paul Greco of mineral resources
1361349668 2015-06-16 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. Fittings and connections at the wellhead are secure. The enclosure gate is locked. Ingress and egress to and from the well and battery is afforded by the gravel-paved driveway which is shared with the referenced residence. The gates and enclosure at the battery are closed, locked, and secure. Load-outs are locked, bull plugged, and secure. Tank hatch lids are closed, sealed, and secure. Process safety equipment (spark arrestor, low-pressure relief valve, and carbon filter) is present. The secondary containment is intact and the drain is secure. The battery enclosure is equipped with two gates as required . The ID signage is missing. The electrical box at the wellhead requires a lock. There is gas leaking from a back-pressure fitting from the production casing. I will contact the owner/operator regarding the deficiencies.
1791090101 2017-10-06 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead pump jack system and associated tank battery located on a larger, residential parcel. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of open, grassy areas adjacent to a wooded portion, and found no signs of impact or degradation attributable to the production system.

The production wellhead consists of a pumpjack system in a 6 enclosure. The tank battery is comprised of 2 100-barrel (BBL) tanks, 1 50 BBL tank, mechanical separator, skid compressor unit, and associated piping and appurtenances enclosed in an 8 enclosure. The system is equipped with production safety equipment required by 1509:9-9-05. The loadout lines are locked and the secondary containment is good. I did not observe any leaks, releases, or problems. The unit ID cannot be seen from outside the enclosure. I spoke with Bob Allen and advised him about the need for adjustment of the sign location.
