0838803980 2017-09-15 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: No locks at the load-lines and no proper sealable lid on the production tank. At the time of this inspection, the noted deficiencies remain. I contacted Mark Smith with PEP and a response was pending at the time of the report filing.
1299513416 2011-03-02 UrbanProduction Wells No Violations No Violations
1371751370 2013-06-20 UrbanProduction Wells No Violations The producing unit consists of a production wellhead in a board-on-board enclosure and an above-ground storage tank (AST) battery also in a board-on-board enclosure. The battery is comprised of 2 100-barrel steel ASTs, mechanical separator, and metering run. Enclosure gates are locked and ID is in accordance with 1501:09. Secondary containment is present at the battery. The wellhead is fitted with a mechanical plunger lift system and is producing. I did not observe any leaks or problems.
1461333705 2016-04-22 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure and associated tank battery is separate board-on-board enclosure. Access to the well and battery is afforded by a short, unpaved access road with concrete apron, which enters from Pettibone Road. The areas surrounding the well are comprised of turf grass with no evidence of impact from the on-going production activity. Fittings and connections at the wellhead and battery were secure and free from leakage. The secondary containment was intact. Load-lines were plugged. I did observe the following maintenance and/or safety deficiencies: The load-line valves were not locked, the tanks were not equipped with lids with functioning seals, and the wellhead enclosure gate lock was broken. The owner must do the following before 05/27/2016: place locks on the load-line valves, fit proper lids with functioning seals on the tanks, and replace the broken lock at the wellhead enclosure. Contact Inspector Stephen Tompkins at 330-907-4646 with questions regarding this inspection.
