At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery is separate chain-link enclosures located at the rear portion of a commercial facility . The enclosure gates were locked. Fittings and connections at both the wellhead and battery were secure. Load-line valves were plugged, locked and secure. The tank lids were closed and secure. Secondary containment was intact and the interior areas were dry and free from signs of leakage or problems. I walked and inspected the areas around the wellhead and battery and found no evidence of impact or environmental degradation relating to the on-going production activity.

Entry gates are locked and secure, at both the wellhead and battery. Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. Tank hatches are closed and secure. At the time of this inspection, I observed the following deficiencies: ID signage is lacking required information. The evergreen screening required as part of site restoration is incomplete (missing on the south side of the wellhead). Warning signage is incomplete. I have notified Bass Energy that these deficiencies must be corrected as soon as practicable or by 06/29/2015. The battery is comprised of 1 140-barrel steel production tank, 1 50-barrel brine tank and separator unit. Both the wellhead and battery are enclosed in separate chain-link fences. The gates and load-outs are locked. ID placard and secondary containment are good. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) is present. No problems noted.

At the time of this inspection, I observed the following: a wellhead with pump jack system located in a screened, chain-link fence which was locked and secure. The fittings and connections at the wellhead were secure and free from signs of leakage or problems. The area around the wellhead consists of asphalt-paved parking for the restaurant that exists on the property. The associated tank battery is located along the southern-most limit of the parking lot, in a separate chain-link enclosure. Gates and load-lines were secure. Signage had been updated and was compliant. I walked the areas around both the wellhead and battery and did not observe any signs or evidence of environmental impact or degradation.

Access to the well and battery is from the party center parking lot which is clear and snow has been plowed. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.
