0109428360 2014-09-16 UrbanProduction Wells No Violations I inspected the location of the spill which occurred on 09/15/2015. Equity-contracted crews are continuing with mitigation and clean-up. Oil impacted topsoil and gravel from the area of the well is being excavated and off-loaded for off-site transport and disposal. The storm sewer has been steamed and no oil is visible. Oil booms are in place in the creek. Additional oil-impacted soil at the wellhead area is being removed. I met with Wade Balser of the Ohio EPA at the location. Wade recommended some additional active removal of some residual floating oil at the creek outfall. The pumping unit is currently shut down pending replacement of the broken rod liner and construction of a dike as requested by the City of Broadview Height Fire Chief.

The natural surface drainage from the area of the wellhead and tank battery is to the north-northeast and I did not observe any signs of gully formation, wash-outs, or other impacts associated with the unit. Enclosure gates are locked. I did not observe any signs of leakage, spills, or other problem conditions.

At the time of the earlier inspection, I had noted that the load-line valves were not secured with locks. At the time of this inspection, the load-lines remained unsecured. The wellhead and battery were otherwise free from evidence of impact or problems. The tank lids were closed and secure. The enclosure gates were locked. The secondary containment was intact and dry. The owner must take the following action by April 15, 2016. Secure the load-line valve with a lock as required. By 1501:9-9-05(E)(3).

1410880593 2014-09-15 UrbanProduction Wells Violations Noted I inspected this location pursuant to a call from Equity Oil at 2:30 PM. A release of oil was reported to be originating from the pumping unit. Upon my arrival I observed the producing unit to consist of a pump jack system in a chain-link enclosure and aboveground storage tank (AST) battery, also in a chain-link enclosure and located 300 feet to the west. The wellhead and battery are both located within a park owned and operated by the City of Broadview Heights. The park is comprised of several baseball and soccer fields. The topography is rolling and surface drainage from the area of the wellhead is to the north where a small, unnamed tributary of Chippewa Creek traverses the property, flowing west to east. Reportedly, the release of oil was caused when the timbers which support the pumping unit collapsed and compromised the valve seal. Oil then flowed north following topography to a storm sewer inlet located 45 feet from the wellhead. When I arrived at 3:15 PM, Equity-contracted crews were beginning mitigation activities. I observed a migration pathway of crude oil originating at the pumping unit and traveling over the gravel road base and grass to the north towards the storm sewer inlet. The crews had commenced removing the top 3 - 4 inches of oil-impacted top soil and had created a temporary stockpile. The crew foreman reported that oil had entered the storm sewer inlet and additional evaluation was incomplete. We inspected the nearest accessible downstream effluent location of the storm sewer, approximately 500 feet northeast of the well location, where the sewer enters a tributary of Chippewa Creek. At the that point, I observed an approximate 2 x 2 area of floating oil which has become trapped behind some debris. Further inspection of the creek several hundred yards down gradient did not reveal any signs of oil migration. I advised the crew foreman to begin installing oil-sorbent booms at the head of the creek outfall and remove the floating oil as much as
1411760005 2014-09-19 UrbanProduction Wells No Violations I inspected the location of this well as a follow-up to a prior release of oil. The oil impacted area around the wellhead has been excavated and removed. Topsoil has been replaced and seeded. Booms remain in the creek outfall area but the water is now free of any oil or oily sheen. Equity reports that the pumping unit will remain idle until repairs are made to the rod liner which was damaged. I will continue to monitor the unit for any other problems.
1419872849 2014-12-29 UrbanProduction Wells No Violations I performed an inspection of this location as a follow-up to a previous incident involving release of oil from the pumping unit which impacted the surrounding area and the adjacent creek. Areas around the wellhead are restored and seeding has been established. A series of booms which had been placed in the creek have become dislodged and are no longer needed. I contacted Tony Carlyle with Equity and advised that the booms could be removed. No further action is warranted with regards to this incident.

The well is equipped with a pumpjack and the valves on the wellhead are open. There was a leak from the wellhead pumping T. The tank battery consisted of 2-100 BBL production tanks, mechanical separator, and a 1500 gallon poly brine tank. The brine tank top was crushed in. The production tank hatches are not sealed properly. The tank valves have bull plugs but are not locked. The well is not properly identified with a clear, legibly marked well identification sign. The well and tank battery were fenced with 8 foot chain link fencing with green slats and 3 strand barb, and the man doors were locked. The lease road is stable and drivable. Will contact the company to repair and maintain the brine tank and add locks to the tank valves. Also include updating the signage. Will reinspect the site in 30 days.
