At the time of the earlier inspection, I had noted that the load-line valves were not secured with locks. At the time of this inspection, the load-lines remained unsecured. The wellhead and battery were otherwise free from evidence of impact or problems. The tank lids were closed and secure. The enclosure gates were locked. The secondary containment was intact and dry. The owner must take the following action by April 15, 2016. Secure the load-line valve with a lock as required. By 1501:9-9-05(E)(3).
0751231839 2012-12-03 UrbanPreliminary Restoration No Violations My visit to the well today was to conduct an Urban Preliminary Restoration inspection. The access road is crushed gravel packed to a depth of six inches. This is a pump jack well and the wellhead is enclosed in chain link fence secured with locks. There is no evidence of oil or other liquids in the wellhead area. The tank battery is enclosed in chain link fence and is secured with locks. There are two 110 BBL storage tanks, caps are closed and secure. There is a vent system on the tanks to include one charcoal filter system in place. The tanks are equipped with a spark arrestor system. There are two pick up lines inside the unit and they are secured with bull plugs. There is one horizontal separator in the unit and one metering run. No violations are found and the well is in compliance. The Urban Preliminary Restoration inspection passed.
0828652628 2015-09-29 UrbanProduction Wells No Violations I inspected this well has a follow-up to a previous inspection. At the time of this inspection, the signage has been updated. The tank lid hatches have been updated and are sealed. The load lines are sealed and secure but require locks. I contacted Tony Carlyle (well tender) regarding the locks. Tony indicated that he is working on a retro-fit lock system and will be deployed within the next 2 weeks.

The well and tank battery are located on a hill behind the house at the referenced address. The well is comprised of a pump jack system in a chain-link enclosure. The battery is comprised of 1 100-barrel steel production tank, 1 100-barrel steel brine tank, mechanical separator, and metering run. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present.

1431627549 2015-05-06 UrbanProduction Wells Violations Noted I was on site to perform an inspection with DOGRM Inspector Shannon Mullins. The tank battery and pumping unit are enclosed with a chain link fence. Upon our inspection we found that the tank hatches on the two 100 barrel tanks are not equipped with the proper sealed hatches and no lightning arrestors. There were no locks on the water or oil load lines. The tank battery entrance gate was locked. The wellhead gate was unlocked. The well is equipped with a pumping unit and an electric motor. There is no bridle on the pumping unit. There is oil on the ground at the wellhead. The oil needs to be removed and contaminated soil removed and disposed of. Electric box was not locked. ID signs are posted but are not in compliance with 1501:9-9-05--( E).
 1) Install proper sealed hatches.
 2) Install locks on oil and water valves at tanks or load lines, and at wellhead gate and the electric box
 3) Remove all contaminated soil and stone and properly dispose of.
 4) Add additional identification sign information as required by 1501-9-9-05  ( E )

1778204097 2017-05-24 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous Compliance Notice for missing locks at the load-line valves. At the time of this inspection, the load-lines are plugged and secure, and the owner has removed the handles per my discussion and subsequent agreement that, since the current load-line valves do not have a hasp to accommodate a lock, then removal of the handle would render the valve safe from potential vandalism. I inspected the remainder of the production elements and found the connections and fittings secure. The tank lids were closed and sealed. The enclosure gates were locked and secure. I walked the areas surrounding the wellhead and battery and found no signs of problems or impacts relating to the on-going production activity.
