Fittings and connections at the well and battery were secure and free from leakage or problems. The tank lids were closed. The secondary containment has a small amount of collected rain water which was clear with no signs of leakage The individual enclosure gates were locked. Production reports were current and up-to-date, but the well was not producing at the time of my inspection. The shared-use access road was in good condition. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation.

On my previous inspection, I had noted that the drain in the secondary containment was open. I had notified the owner/operator and the issue was resolved. The wellhead and battery enclosures are locked and secure. Load-lines are locked. Process safety equipment is present and in good condition. The fittings and connections are secure and free from signs of leakage. The owner is working on addendum signage to meet urban requirements.

At the time of my inspection, the area is covered with several inches of snow. Access to the wellhead and battery are via the shared residential driveway which has been plowed and is passable. Valves and connections at the battery are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.

The production wellhead is fitted with a mechanical plunger lift system and is currently producing. The pumpjack unit is enclosed in an 8-foot board-on-board enclosure which is locked. The tank battery consists of 1 100-barrel steel tank, 1 50-barrel poly tank, separator, and 2 domestic meters. The battery is enclosed in a chain-link enclosure. Safety equipment (carbon filter, spark arrestor, low-pressure relief valve) is present. The secondary containment is present and in good condition. No violations noted.

At the time of this inspection, I noted the following: a wellhead with plunger-lift system and associated tank battery in separate board-on-board enclosures located on a residential parcel. The enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. . Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment drain was secure. There was some standing water in the containment area from recent rainfall. The water was clear with no signs of floating oil or sheen. The gravel-paved access road is shared with the residence , and was level and dry. I noted that the carbon filter canister, which had been recently changed out, was showing some signs of corrosion and will need to be replaced. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity. I notified Don Nething with Ohio Valley about the need for maintenance at the carbon filter.

The well and battery are located behind a single-family residence on Royalwood Road. Access to the elements are via the shared driveway. The well is comprised of a mechanical plunger lift system is a board-on-board enclosure. Fittings and connections are secure and free from signs of leakage. The surface casing is plumbed to a motor valve. The enclosure is locked and ID is present but is lacking complete required information. The battery is comprised of one 100-barrel steel production tank and 1 50-barrel brine tank in a separate board-on-board enclosure. Process safety equipment is present. Secondary containment is intact and dry. The areas surrounding the well and battery are comprised of meadow grass. Some portions are being used for equipment storage by the land owner. I did not observe any signs of impact from the on-going production activity. I advised the well owner that signage requires updating. The owner indicated that new signage is in process.

1601782082 2012-11-06 UrbanPreliminary Restoration No Violations I visited well site with supervisor Tom Hill. Well site has paved entry for first 100 to gate. Wellhead is a pump jack and is surrounded by board on board fence. There is a dike around wellhead and the area is clean and neat with no obstructions in the area. The tank battery is surrounded by board on board fence with two 110 barrel storage tanks and the dike will hold sufficient liquid for one tank. There is one 50 barrel brine tank, a separator, and a metering run. There are two load lines secured with locks. The site lacks identification signs at this time. I placed a call to the well owners and signs will be erected within two weeks. Urban preliminary restoration not approved pending installation of signs.
