1097278566 | 2017-10-06 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead plunger lift system and associated tank battery located in an otherwise undeveloped parcel. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of wild vegetated areas, and found no signs of impact or degradation attributable to the production system. I notified Duck Creek Energy about the need for replacement of the carbon filter element. |
1321850975 | 2018-08-20 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. At the time of this inspection, I noted the following: The wellhead is comprised of a plunger lift system is a small, triangular chain-link enclosure. The battery is comprised of 2 100-barrel stock tanks, separator, and Dominion metering run inside a separate chain-link enclosure. Fittings and connections at the wellhead were secure and free from signs of leakage. I did not observe any evidence of impact relating to on-going production activity. The well was in active production. |
1520140668 | 2017-02-10 | UrbanProduction Wells | No Violations | I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery located on a large parcel wooded parcel within a commercial and residential area. Fittings at the wellhead including flow-line connections and fittings were secure and free from leakage. The tank battery was comprised of two 100-barrel steel production tanks with mechanical separator. The secondary containment was present and intact. The visible system connections were secure and free from leakage. The well is bordered on the west by a steep ravine that drops to stream drainage. I walked the areas surrounding both the wellhead and battery with a focus on the down-gradient area. I did not observe any signs of erosional deficits or impacts associated with the producing unit. The access road was mostly well drained and stable with no signs of wash-out or other drainage problems. |

2099005630 | 2006-03-16 | Drill / Deepen / Reopen | No Violations | Capstar Drilling Rig #3 is on site drilling surface hole. I spoke with Dave Mansbury with Duck Creek and requested fencing around the drilling pits and either silt fence or straw bales across an old lane south of the well. The old lane will collect and direct surface runoff from the location. Duck Creek complied with both requests. |
