0473710062 2017-04-27 UrbanProduction Wells No Violations I inspected this producing well as a follow-up to a previously-issued Compliance Notice for the following deficiencies: storage tanks lacking sealable lids, ID signage missing required information, and missing locks on load-line valves. In addition, I had noted that there was some residual oil from a spill inside the battery. At the time of this inspection, I noted that the previously-noted deficiencies had been corrected and the residual oil spill was remediated. I did note that the signage had become faded and needed maintenance. I inspected remainder of the production system and surrounding areas and found no signs of problems. The fittings and connections were secure and free from signs of leakage, spills, or other issues. I walked the areas around the wellhead and battery which were comprised of wooded areas and found no signs of impacts or problems. I notified Tony Carlyle with Equity Oil regarding the need for sign maintenance.

The production wellhead is fitted with a pumpjack system and is currently producing. The pumpjack unit is enclosed in an 8-foot chainlink enclosure which is locked. The tank battery consists of 3 100-barrel steel tanks, and separator unit. The battery is enclosed in a chainlink enclosure. The secondary containment is present and in good condition. The tank battery is shared with the Sokol 2 Unit. No violations noted.
0585507319 2015-05-22 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. The well is comprised of a pump jack unit in a chain-link enclosure which is locked and secure. At time of this inspection, I observed the following deficiencies: two of the four production tanks do not have hatch lids capable of sealing, load lines are not protected with locks, ID signage is present but lacking required information, locks are required on electrical panels. In addition, there is some surficial oil staining inside the battery near the load-outs and inside the wellhead at the back-pressure-valve. I did not observe any free oil and there is no active leaking at the time of this inspection. The stained soils, gravel and other surfaces should be kept reasonably free of oil contamination. These tank lid and lock deficiencies must be corrected as soon as practicable or by 06/29/2015. Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

Fittings and connections at the well and battery were secure and free from leakage or problems. Load lines were plugged and locked. Tank lids were closed. The secondary containment was intact and free from standing water, or signs of leakage of petroleum fluids. The individual enclosure gates were locked. Bonding and insurance were current. I noted that production reports for calendar years 2014 through 2016 were missing and I called Elaine King with Equity Oil. A response was pending at the time of this report filing. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation.

The well and tank battery are located near the front entrance to the camp facility at the referenced address. The well is comprised of a pump jack system in a chain-link enclosure. The battery is comprised of 3 100-barrel steel production tanks and mechanical separator. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. The access road is good, passable condition with no washouts or other erosional deficits or signs of impact. The battery is shared with the #2-2072 well located further south within the facility.

The battery is comprised of 3 140-barrel steel production tanks, and 2 separator units. The elements are enclosed in separate chain-link enclosures. There is adequate secondary containment. The signage at the battery is almost unreadable and needs remedied. The ID placard identifies this unit as the Sokol #1 (#21840). That well was plugged and abandoned previously. I will contact Equity about the revision. Urban drilling process safety equipment is present. The load outs are locked and bull-plugged. No other problem s noted.
