0984282641 | 2017-10-06 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead plunger lift system and associated tank battery located adjacent to a cemetery parcel. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I did note that 210-barrel stock tank was showing some areas of severe corrosion and in need of repair/replacement soon. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system. I notified Duck Creek Energy about the pending need for repair/replacement of the 210-barrel stock tank. |

1425326430 | 2015-03-02 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. The wellhead and tank battery are located in a small undeveloped area on the east side of Brecksville Road, adjacent to the Maple Shade Cemetery. The wellhead is comprised of a plunger lift system is a small, triangular chain-link enclosure. The battery is comprised of 1 210-barrel production tank, 1 100-barrel brine tank, separator, and Dominion metering run inside a separate chain-link enclosure. Fittings and connections at the wellhead are secure and free from signs of leakage. The production tank is showing areas of moderate to severe corrosion but is intact at the time of this inspection. I did not observe any evidence of impact relating to on-going production activity. |

At the time of this inspection, I noted the following: The wellhead is comprised of a plunger lift system is a small, triangular chain-link enclosure. The battery is comprised of 2 100-barrel stock tanks, separator, and Dominion metering run inside a separate chain-link enclosure. The former 210-barrel stock tank that was corroded had been replaced. Fittings and connections at the wellhead were secure and free from signs of leakage. I did not observe any evidence of impact relating to on-going production activity. |

1789498735 | 2017-02-10 | UrbanProduction Wells | No Violations | I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery in separate chain-link enclosures located on a parcel otherwise undeveloped. Fittings at the wellhead including flow-line connections and fittings were secure and free from leakage. The tank battery was comprised of one 210-barrel steel production tank, and one 100-barrel steel brine tank with mechanical separator. The battery is shared with the #2 wells(#2-1797). The secondary containment was present and intact. The visible system connections were secure and free from leakage. Load lines were plugged and secure. The well is surrounded by tall, grassy landscaping. The battery is bordered by level, landscaped areas and some wild vegetation. I walked the areas surrounding both the wellhead and battery and I did not observe any signs of erosional deficits or impacts associated with the producing unit. The access road was mostly well drained and stable with no signs of wash-out or other drainage problems. |
