1020902829 2012-10-23 UrbanPreliminary Restoration No Violations My visit to the well site today was for an Urban Preliminary Restoration Inspection. This is a rabbit well and the wellhead area is neat clean and free of debris or liquids. The well head area is enclosed in chain link fencing and is secured by locks. The tank battery is enclosed in chain link fencing and is secured by locks. There is one 140 BBL storage tank and one 50 BBL brine tank. Lids and caps are closed and secure and a vent system is in place to include a charcoal filter. There are pick up lines inside the battery and they are secured with locks and bull plugs. There is a metering run and one vertical separator located in the tank battery. Urban Preliminary Restoration Passed.

1230802430 2014-10-09 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is comprised of a mechanical plunger lift system in a chain-link enclosure. Valves and connections are free from signs of leakage and appear functional. Areas surrounding the well area well maintained and free from signs of impact. The well is producing and production reports are current. Signage is present. The tank battery in a separate chain-link enclosure. The fittings and connections within the tank battery are free from leaks. The secondary containment is intact and the interior is free of signs of leaks, spills, or other problems. The load-outs are bull-plugged. The gate is secure.
1380292004 2013-09-27 UrbanPreliminary Restoration No Violations The producing wellhead is fitted with a mechanical plunger lift and is located in a locked chain-link fence. The associated tank battery is comprised of 1 100-barrel steel production tank, 1 25-barrel poly brine tank, and separator. The battery is located in a separate chain-link enclosure. Load-outs are bull-plugged and locked. Secondary containment is good. The battery needs safety equipment (LPR valve, spark arrestor, carbon filter). I will discuss the missing items with Ralph Gregory of Bass Energy.
1815926488 2015-11-16 Final Restoration No Violations I inspected the former location of this producing unit as an evaluation for the completion of tasks necessary for final restoration. The former locations of the well, well pad, and tank battery have been graded, seeded, and restored. Access to the well and battery which were afforded by a short, unpaved access road which has been reclaimed. The flow lines and sales line were removed per report of Bass Energy. No further action is needed and Final Restoration passes.
