0483466642 2010-04-15 Orphan No Violations Meeting at Westlake Fire Department. With Fire Chief Kraus, Building Department Donald Grayem,

Columbia Gas Amy Konalik & Brian Barclay, Suzanne Offill Manager & James French Maintenance For Barrington Apartments. Mr. Brarclay explained the gas samples that were taken at the apartment building , the results showed it was not their line gas. He also stated that the gas will not be turned on until the reading in and around building #1543 are at zero % LEL.I explained I surveyed area with metal locator found no metal which would indicate a possible old gas well. I advised to look into putting a power ventilator that would vent under the slab.

I advised Fire Chief Kraus to let me know when the system is installed.

0931942618 2010-04-12 Orphan No Violations Mr. Tomcho of Columbia Gas had taken samples of their gas from supply meter and samples of the unknown sources( from south end, front, and behind the apartment building #1543 on the east side of property.) Columbia gas checked perimeter of the building and inside building. Reading from 17-30% LEL in the lower level apartments and on the outside 17 to 25 % LEL (along the southwest, south and southeast perimeter of the building). Then check along their gas line right of way along eastern property line. Found a high reading (60%LEL) approximately 35 north from southeast corner then 15 east of building in same area as the supply line for the building. I advised Sam French Maintenance Supervisor to hire someone to dig in that area. I gave Mr. French Ron Gibsons P# to call. I surveyed site with magnetic locator found no steel casing. Then I used computer looked up area found one well on the west side of property P#1469 that was a shale well and plugged to surface with grout on 8-31-88.I located well with measurements from the plat on file which locate close to apartment building on the westside of property. There is no field evidence of any leakage from this well.
1282242421 2010-04-13 Orphan No Violations Contractor (Ron Gibson ) on site to dig behind apartments at the spot where Columbia Gas had detected a high reading(60%LEL) on their Meters. Dug down found a square concrete walled box with sandstone lid. Removed top no gas well found it appears to be a transfer box for fluid. Found 6 lines one going west toward the apartment building. Columbia tested for gas 9% gas at opening of tile . I advised to dig at the wall of the apartment which was built on slab, NO BASEMENT. Mr. French called Roto Rooter to run camera into the tile. this was done on 14th of April. The tile was collapsed approximately 3 from building. Columbia Gas had called for a crew to excavate their line closet to the excavation dug by Mr. Gibson. Columbia Gas dug p their line no leaks found. Ron Gibson is to vent the 6 tile with plastic pipe.

The term Fermentation gas that its from Decaying material. Then showed him the platted location of the plugged well (P#1453) which is on the west side of the Apartment Complex. Showed him the apartment building which was affected by the odors. also pointed out the power ventilator which was installed on the side of apartment building #1543.
