0145962969 2017-08-28 UrbanProduction Wells Violations Noted I inspected this producing well location as a follow-up to a previous inspection in which I had noted the following: The wellhead enclosure was overgrown with vegetation restricting access for maintenance and inspection, there was no compliant identification anywhere on the well or battery, and there were elements from an abandoned battery system located across a small drainage, north of the existing system. My subsequent communication with the well owner indicated the existing landowner had barred access to the production system, precluding the well owner from performing maintenance. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner must remedy by performing the following: place proper identification at the wellhead and battery, remove the vegetation overgrowth, and remove the abandoned tank battery elements, including any unused flowlines, and restore the area. These activities must be completed by 09/29/2017. Contact Inspector Stephen Tompkins with any questions regarding this inspection.

0269538415 2017-06-19 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger-lift in a small chain-link enclosure located in a recreational field behind a church. The interior of the enclosure had become heavily overgrown with vegetation rendering inspection difficult. The fittings and connections appeared to be secure. The associated tank battery is located at the eastern fringe of the same field. The interior of the battery was similarly overgrown. The existing ID signage was unreadable. The well enclosure was not locked. The current landowner met with me during my inspection and reported the presence of former tank battery production equipment located east of the current battery location. The observed equipment was comprised of a mechanical separator, a small (less than 25 barrel) steel, stock tank, and associated piping and fittings in a chain-link enclosure, with no identfying markings or signage. Reportedly this equipment was abandoned by the owner operator and has become derelict and unused. I contacted the owner operator, who indicated that the landowner had blocked access to the well and battery due to a dispute regarding whether an active lease agreement was in place. Subsequent to this discussion, I contacted the landowner and advised that the dispute must be resolved between the landowner and well owner. We agreed that I would follow-up in one month.

The well is currently in production with a mechanical plunger-lift and valve assembly inside a unlocked chain-link enclosure. The tank battery is comprised of 1 50-barrel production tank and separator unit also in a chain-link enclosure. ID placard is present at the battery but is needed at the wellhead. There is no clear lease road. The unit is in a field behind a church. Annual production reports are current. The unit requires locks and safety equipment in accordance with 1501:9-9-05E. These items should be remedied within 30 days.

The tank battery consists of 1 50-barrel steel tank and separator. The battery is enclosed in a chain-link enclosure. The secondary containment is present and in good condition.The enclosures are not locked. I contatced Matt Pettigrew about the need for locks on the enclosures. Im waiting to hear back from him.

1500853944 2018-05-24 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a compliance notice issued previously for the following: missing identification, overgrown vegetation in the wellhead and tank battery enclosures, and unused and derelict equipment from a previous tank battery location. During subsequent discussions with the well owner, they had made pledges to rectify the defficiencies. At the time of this inspection, the vegetation had been removed, however, the signage remained missing, and the derelict equipment remained.
1733124277 2018-08-22 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous Compliance Notice issued for the following deficiency: the interior of the wellhead enclosure and the battery enclosure were significantly overgrown with vegetation. I had subsequently spoken with representatives of Petrox and they had pledged to remedy the deficiencies pending a resolution to access problems with the land owner. Since that time, the interior remained overgrown. I contacted Petrox and they indicated they would address the vegetation.
1941113450 2018-03-09 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following issues: there was no compliant ID signage, the interior of the wellhead and battery enclosure had become overgrown with vegetation. In addition, there were elements of a previous tank battery system left behind when the battery was relocated to the west side of the stream the traverses a portion of the property. There had been an on-going dispute between the landowner and Petrox regarding mineral and access rights. At the time of this inspection, I met with Pastor Sepkovich who indicated that a meeting had occurred between the church and Petrox, which resolved the disputed access, and as such, Petrox had planned to resolve the deficiencies. At the time of this inspection, I noted the following: fittings and connections at the well and battery were secure. Enclosure gates were locked. The interiors of the enclosures were free from vegetation overgrowth. The ID signage remained missing, and the discarded battery equipment remained. I contacted Duane Clark with Petrox and he reported that the ID signage would be fixed as soon as possible and they were working to get the unused equipment removed as soon as the weather was better.
