0559006221 2010-03-15 Plug / Plug Back No Violations On site to witness plugging of this orphan well. The plan today is to spot 660 of class A cement with 2-3/8 upset tubing. The bottom of the hole is 880. Petroset broke circulation and mixed and pumped 78 sacks of cement. Circulation was maintained throughout the job. The well will be topped off tomorrow with 9 sack grout and the 5 casing will be removed if possible.

1236794872 2009-02-12 Orphan No Violations on site with company whos planning to demo building then to rebuild vent running up along the south wall of building no gas reads. Advised to find well then during building to reroute the vent. Advised Micheal Glenn to let me know when building is to be demo.
1421428533 2015-01-16 Final Restoration No Violations I inspected this location for evaluation of completion of the requirements for final restoration. The well was plugged and abandoned in accordance with OAC 1501:9-11. At the time of my inspection, the well is located within a subsurface vault structure with a 24-inch cast iron, bolt-down access lid. 12 inches to the south of the lid is a 10-foot vertical vent riser consisting of a 3.5-inch diameter galvanized pipe with rain cap. The lid and vent stack have been incorporated into a typical curb island within a larger asphalt-paved parking lot. Impacts relating to the plugging activity have been restored. No other evidence or remnants of the former well are visible. The site has met the requirements for final restoration.
