1449596449 2015-12-08 Orphan No Violations I inspected this well location pursuant to a complaint about gas odor which may be originating from a historic well which is recorded to exist in this vicinity. At the time of this inspection, I met with the complainant along with City of Mayfield Service Director Doug Metzung. During the inspection, I noted occasional gas odor but no meter-detectable readings. An area of soft ground was observed and investigated which revealed the presence of a solid object. Hand-digging revealed a 2-inch diameter steel pipe which had been cut and capped with a PVC cap, at some time in the past. After some consultation, the Service Director called on crews to excavate the location. Excavation was executed with a small, rubber-tire back-hoe. The excavation revealed a 10-inch diameter casing which was in-filled with cement, 2 feet below grade. Sealed concentrically within the 10-inch casing is a 2-inch diameter pipe, also in-filled and sealed with cement. The 2-inch diameter pipe which had been previously discovered, traverses south. Gas readings near the inferred wellhead were observed to be varying from 0 to 30% LEL. The well is located within a larger cement structure, inferred to be a vault of some type. The top of the vault structure was noted to be 2 to 3 feet below the surrounding grade. The service crews re-buried the suspected well, and will leave a marker per my advice. We agreed to discuss potential options and strategies going forward.
1462368992 2016-05-03 Orphan No Violations ON THIS FOLLOW-UP INSPECTION, I COULD NOT LOCATE THIS WELL AT THE RECORDED COORDIANTES. I SAW NO EVIDENCE OF A WELL, IE: CASING, FLUID OR GAS BUBBLING ON THE GROUND. HOWEVER, THERE IS A SLIGHT NATURAL GAS ODOR IN THE SUSPECT AREA. WE WILL RESEARCH OUR RECORDS FURTHER TO TRY TO DETERMINE THE LOCATION OF THIS WELL.
