Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0452974102 2014-06-23 Orphan No Violations I inspected this location pursuant to previous reports of gas odor. The gas odor complaints seem to coincide with the recent municipal construction of a walking path and pedestrian tunnel. I walked the area and could not detect any odors or detect gases with hand-held meter. Continued periodic surveillance is advised.
1394643060 2014-03-03 Field Inspected, Well Not Found No Violations I inspected the area where this well is suspected to be pursuant to a complaint. The well may be in the area where some recent earth work has been performed as part of a trail project by the City of Mayfield. A faint gas odor was apparent from time to time, but a high enough concentration to register on the gas meter. Owing to the heavy snow cover, attempts to locate the well were not successful. I will follow-up when snow melts and a better assessment can be made.
1900054990 2015-12-08 Orphan No Violations I inspected this well location pursuant to a complaint of gas odor which has been noted at various times near the walking trail area near here. At the time of this inspection, I noted the following visible elements: a 12-inch steel casing riser which extends 12 inches above grade, in an otherwise isolated, and wooded area. Sealed concentrically with cement within the casing is a 2-inch diameter pipe, the annulus of which was open to the extent that is visible. The top of the 2-inch pipe terminates within 3 inches of the larger casing. I recorded sustained readings of 10% LEL at the opening of the 2-inch pipe. The inferred wellhead is located within a larger cement pad, or vault-type structure, the purpose of which is not known. Following inspection, I spoke with a grounds and service professional (Brian Smith) from Progressive Insurance, which owns the property on which this well is located. We agreed to meet and discuss my inspection and management options.
