0492057334 2019-02-25 Production Wells No Violations On the day of my inspection I found that there were no changes at the well site. I called Dale Smith to ask if he had any new information on their plans to produce the well. Dale said that once he looked into it he discovered that this well was not in his area of responsibility. He said that Chris Belch was responsible for that well and he passed my concerns on to him. I called Chris and he confirmed that there was some dispute between MFC and M&R Investments (Diversified) concerning the gas sales line. Chris said that there has not been any recent discussion about the well. He said that he would discuss the situation with his administration and get back to me with a tentative plan.

0892614733 2013-09-30 Plug / Plug Back No Violations On the day of my inspection Kilbarger Drilling Rig 1 reported encountering an obstruction at approximately 6541 when attempting to run drill pipe in for the bottom plug. The pipe was hung briefly, but they were able to get it free. At that point they lost circulation due to the open end of the drill pipe being plugged. Petroset made and unsuccessful attempt to pump on the drill pipe to dislodge the material. Kilbarger Drilling then made the decision to trip out of the hole, clear the plugged drill pipe and run back in.

1376423530 2013-08-13 Casing String Witnessed No Violations On the day of my inspection I found that FMS Drilling had drilled a 17.25 conductor hole on fluid to 62. They ran 60 of new 11.75 conductor casing. Petroset broke circulation and pumped 90 barrels of cement. Good circulation was maintained throughout the job, with 7 barrels of cement returns.
1376578893 2013-08-15 Drill / Deepen / Reopen No Violations I received notification from Scott Kees of NGO, that a fatal accident occurred on the drilling rig at or around midnight. He reported that FMS Drilling was at a depth of 289 drilling on air when they encountered a high volume of water. According to Kees, FMS Drilling was in the process of switching to fluid drilling when the derrick hand fell from the finger boards sustaining fatal injuries.
