1139315991 2016-11-15 Production Wells No Violations I was contacted again by Mark Haynes. He expressed concerns about the access road situation. I advised Mr. Haynes that the use of his easement to access the well was a legal matter between him and Luqman Yaccub. He also claimed that the landowner had expressed concern about contamination in the stream that runs through his farm. I went back out to check the stream again. I found no contamination on the ground and no evidence of excavated soil between the wellhead and the stream. The water was clear, and the steam was abundant with minnows. I tested a sample with a Quantab and found no chlorides present in the water. I will continue to monitor the stream.

1479139533 2016-11-14 Production Wells Violations Noted I was contacted by Mark Haynes who owns the neighboring farm to this lease. Mark claimed that Elephant Exploration was pumping crude oil into a hole and it was running into the creek. He also claimed that they were blocking his easement with equipment. On the day of my inspection I found that the access road was also used as a driveway to access the farm where the well is located, and to get back to Mr. Haynes farm. The main part of the road was in good repair and covered in gravel. There was no gravel after it split off past a barn and through a gate to the well, but the road was still smooth and easily passable. The well was equipped with a natural gas powered pumping unit that was in operation at the time of my inspection. There were multiple areas of freshly disturbed soil around the well site. There were two areas that off to either side of the wellhead where steam pits would have been located, and there was an area extending straight out from the wellhead. I found no staining of crude oil at either of the suspected pit sites. There was some minor staining in on the surface of the disturbed soil in front of the wellhead, but the soil was not viscous and there was no odor. I also found no evidence that any oilfield related fluids had been running from the well site to the creek. There was a new 8.625 inch wellhead on the well. The stuffing box and the 4.5 inch wellhead hammer ring were leaking crude oil down the side of the wellhead and onto the ground. I called Luqman Yaccub of Elephant Exploration, who claimed that they had been out recently to service the well. He said that they did have two pits dug; one for steaming the tubing and the other as an emergency backup in the case the well started to flow. Luq claimed that the only thing in the pit was paraffin that was covered up with 10 feet of soil. I told him that they must use a liner when they steam tubing. I also told him that they need to repair the leaks at the wellhead and clean up the oil. The tank ba

The site had been subjected to steady rain the previous day, and I found no signs of crude oil contamination in any of the recently disturbed soil around the pumping unit. I also walked down to the stream and found it to be running clear with no signs of crude oil or paraffin. This violation is resolved.
