0520250176 2019-05-24 Production Wells No Violations On the day of my inspection I found what appeared to be the remnants of a flow line leak on the way back to the well. I found a stretch of disturbed soil extending approximately 200 feet along the access route to the pumping unit, and in line with the pipeline markers. Upon closer investigation I found that there was a petroleum odor that permeated the general area, and there were several areas of oil stained soil. I also found several small patches of standing crude oil on the surface. The issue was compounded by the fact that the impacted area was in the middle of a water outlet course between step ponds. The area was mostly dry, but there was approximately 1/2 gallon per minute of water draining through the area downhill through an isolated patch of wetland. I met Jason Blocker at the site and we determined that no crude oil had went through the patch of wetland. The oil was primarily located uphill at the original leak site. We called Chuck Woods with M&R Investments and he sent a crew out to the site. Dale Smith came out with absorbent pads and BioSorb. Dale said that his pumper reported a gas only flow line leak in the fall of 2018 and he was never notified of any crude oil release. Dale said that he had not been back to the site since his crew had made the line repair, and he was not aware of any need to notify ODNR. Jason directed the crew to install two underpasses with earthen berms and plastic pipe to allow the water to pass through while collecting any residual oil. They also put pads down behind the berms and Dale spread BioSorb on the areas where we found oil in the tall grass. The actions performed today were only temporary, and Dale agreed to bring a crew out after the holiday weekend to conduct a proper clean-up and remediation of the site. I will continue to follow up and coordinate with Jason Blocker.

The tank battery consisted of one 100 barrel tank, one 30 barrel poly tank, one separator and an earthen dike. Identification was posted and I found no leaks or spills. The tank battery is shared with permits 6819 and 6823.

2110239242 2019-06-07 Production Wells Violations Noted On the day of my inspection I found that nothing more had been done at this site since my last visit on 5-24-19. On that day I met Dale Smith at the site to install temporary control measures with earthen berms, absorbent pads and BioSorb. I was assured that M&R would have a crew out to conduct a proper remediation after the holiday weekend. It has been two weeks since the initial response, and nothing has been done besides changing out absorbent pads. There was oily sheen in the water that had collected behind the berms and the pads were dirty. The well owner shall remediate the affected area, to include grading and seeding, within 10 days of the issuance of this compliance notice.
