0113891295 2014-06-13 Production Wells No Violations FOLLOW UP TO CA #1400076865: On the day of my inspection I found that the entire tank battery had been reconstructed to include a substantial dike. The number of storage tanks had been reduced to one 100 barrel and one separator. This violation is resolved. I contacted Scott Kees to give authorization to put this well back into production.
0213207320 2014-05-23 Production Wells No Violations The area that was affected by the spill has been cleaned up, graded out, seeded and covered in straw. I found no residual pockets of crude oil and I found no evidence of further run off into the stream. The absorbent pads and booms have been removed from the stream. I did find some puddles of oil contaminated rain water in the grassy area approximately 25 from the where the spill occurred. I contacted NGO to get someone to meet me out here with some peat moss to soak it up.

1399666272 2014-05-09 Production Wells Violations Noted I received notification from Scott Kees of NGO that they had a release from a 210 barrel tank that escaped the containment dike. He reported that approximately 3 to 5 barrels of crude oil made it into an intermittent stream. When I arrived at the site I found that the 210 barrel tank was in a dike that was less than 6 inches deep. The inside of the dike was contaminated with crude oil. The contaminated soil leading from the dike to the stream had been taken up and stockpiled for treatment. I walked the stream and found that a sheen was present on the surface of the water. I found no actual pockets of oil in the stream. NGO had a crew on location placing absorbent pads and booms at various intervals along the stream. They also had a crew at the intersection of the stream and White Eyes Creek that had booms in place and a vacuum truck to remove any pockets of crude oil. The Ohio EPA was notified of the incident. NGO is to continue with clean up efforts throughout the weekend. I will inspect the site on 5/12/14.
1399991604 2014-05-12 Production Wells No Violations On the day of my inspection I found that NGO had addressed any pockets of crude oil collected along the edge of the intermittent stream. I did not observe any oil and I did not observe any sheen on the surface of the stream or White Eyes Creek. At the time of my inspection a clean up crew was on location spreading peat moss inside of the dike. They also spread peat moss and placed absorbent pads on any remaining pockets of crude oil around the tank battery. I instructed them to line the path to the stream with absorbent pads to prevent any residual contamination from entering the waterway. I met with Scott Kees of NGO to discuss their intentions for remediation of the tank battery and containment dike.
1400076865 2014-05-14 Production Wells Violations Noted On the day of my inspection I found that the inside of the dike, and the areas of contaminated soil outside of the dike were covered with peat moss, but they were not cleaned up yet. I contacted Scott Kees to urge him to get a crew out right away to take up the contaminated soil. I tested a water sample from the intermittent stream for chlorides. I collected the sample approximately 50 feet downstream from the point of entry. The water tested at 28 ppm for chlorides. I did not observe any sheen on the water and I did not observe any impacted wildlife. I informed Scott Kees that NGO is required to construct an adequate containment dike before putting the well back into production.
1400248233 2014-05-16 Production Wells No Violations On the day of my inspection I found that the area of contaminated soil below the dike had been addressed. The stockpiles of soil had been treated with lime and peat, and they were spread out along the lease road. The pockets of oil that had collected in rain water were cleaned up and the area had been graded out to prevent rainwater from pooling. The entire 37 x 42 area was graded out and covered in straw. There was a crew on site making a final sweep down the stream to remove any remaining absorbent pads and booms.
1402666144 2014-06-13 Production Wells No Violations FOLLOW UP TO CA #1399666272: On the day of my inspection I found that all visible areas of contamination had been cleaned up and/or removed from the location. All of the absorbent pads and booms had been removed. I found no visual evidence of crude oil in the soil around the well site, nor did I find any visual evidence of crude oil in the intermittent stream that was impacted by the spill. This violation is resolved.
