0113709846 2019-05-09 Production Wells No Violations Jason Blocker called me this morning to report that M&R was mobilizing equipment to start the clean-up at this site. When I arrived at the site I found that they had one dump truck, one mini-hoe and one back-hoe working at the site. The crew had 70% of the stream bed cleaned down to bare soil. They were still working on the hillside where the leak had occurred, and the area just below. They were spreading gypsum on the areas that were clean. Chuck Woods showed up while I was at the site with a truck load of straw bails. He said that they were going to treat the area and then put down seed and straw once they had all of the oil out. I also met with the Woodbury wildlife area manager while I was at the site. He was aware of the clean-up operations and was satisfied with the progress.
0470575581 2019-02-26 Production Wells No Violations On the day of my inspection I found that there were no major changes at the site of the flow line leak. The area was very muddy and difficult to access. There were some fresh excavator tracks but no signs that any of the affected soil had been removed and hauled away. The oil was still suspended in peat and I found no signs that it had migrated into the nearby stream. The diversion ditch was effective at keeping the moving rain water and snow melt away from the oil saturated peat. I called Chuck Woods with M&R Investments to get an update. He said that it was far to muddy to bring in excavation equipment, but he has had a crew in with a mini-hoe to keep the diversion ditch clean. He also said that they have been adding more absorbent material as needed. Chuck said that they are keeping an eye on it, and they will be in to finish the clean up as soon as the area dries out some. He also said that they plan to move the tank closer to the well, and use the line that failed solely for gas transmission to avoid future issues.

1549560362 2019-02-06 Production Wells No Violations I was contacted by Chuck Wood with M&R Investments to notify me that they had discovered a flow line leak between the wellhead and tanks. Chuck said that they already had a crew at the site and they had the leak contained. I met with Jason Blocker at the site to follow up. It was raining and it had been raining all day. The site was very wet. We found that the line had been repaired and the area of oil impact had been covered with a thick layer of peat moss and BioSorb. They also had three hay bail berms in place. There was still oil laying on the ground but it was contained and suspended. Jason was in contact with Chuck and he said that they will need to wait for the heavy rains to pass so the area can dry up a little before they bring equipment in to excavate the affected soil.
1558365368 2019-05-20 Non Well No Violations On the day of my inspection I met with Jason Blocker to follow up on the flow line leak that M&R was in the process of remediating. Approximately 95 percent of the affected area had been cleaned up, graded out and covered with straw. The only residual oil that we found was on the slope and the base of the hill where the actual leak had occurred. We called Chuck Woods to request that he send a crew out to put down oil absorbent pads. The site is far too wet and muddy to do any more digging. We asked Chuck to have his pumpers monitor and change the pads as needed until there is no more oil.
1790469751 2019-05-07 Production Wells No Violations On the day of my inspection I visited the site of the flow line leak with Jason Blocker. We found that there had not been any more work since winter. The water diversion was still in place and working good. The original stream course was still filled with oil saturated peat and straw bails. There were some small pools of standing oil scattered throughout the affected portion of the stream bed. Jason and I did not find that any of the oil had migrated beyond the stream bed into any of the moving water. Jason said that he was gong to reach out to M&R Investments to get a plan of action.
