0569212434 2019-03-28 Production Wells No Violations I had a previous incident with this well in January of 2018 where I found the tank was leaking from a rust hole. When I drove by today I noticed that there was a dark streak running down the side of the tank so I stopped to investigate. I found that the tank was empty and the dark streak was a stain from the previous leak. There was no oil or brine in the dike around the tank. The soil was dry and clean.
1235826927 2018-01-23 Production Wells No Violations On the day of my inspection I noticed a dark streak running down the side of the 100 barrel tank while I was driving by. Upon closer investigation I found that the tank was leaking from a rust spot approximately 7 to 8 feet high. There were streaks of yellow paraffin/crude oil down the side of the tank. The dike was full of water, likely from the recent snow melt. I tested the water in the dike for chlorides with a Quantab and I found none. There were some small floating patches of the thick yellow substance and there was a slight sheen on the water that originated from the base of the tank under the leak. The tank was in a shallow earthen dike and the water was contained. I called Jeff Butler, pumper for NGO/Diversified to report the issue. He said that he would get report it to his supervisor and they would get out to address the problem. I suggested that they get a water truck out there to pull the fluid out of the tank and vacuum as much water out of the dike as they can. Once the fluid is removed they can work on the affected soil/gravel in the dike.
1333952513 2015-09-14 Non Well No Violations On the day of my inspection I found that the contaminated soil had been hauled away and the new top soil had been graded out, seeded and covered in straw. The 5 barrel drip tank had been removed. I found no residual crude oil contamination at the Pew-Cyclops meter run.
1514573581 2013-10-31 Production Wells No Violations On the day of my inspection I found that the well head was equipped with a plunger lift system. The tank battery consisted of one 100 barrel stock tank and one 30 barrel poly brine tank, one separator and an earthen dike. Proper identification was posted and there were no leaks or spills. No violations at this time.
1627672969 2018-01-25 Production Wells No Violations On the day of my inspection I found that the water had been vacuumed out of the dike and the tank had been drained. There were no signs of residual oil or paraffin in the dike and there was not any fluid leaking from the hole in the tank. I called Dale Smith the field supervisor for Diversified to confirm the actions taken to address the issue. Dale said that they had a water truck out there the same day that I had called Jeff Butler.
1650688000 2015-09-08 Non Well No Violations On the day of my inspection I found that NGO had excavated the contaminated soil at the Pew-Cyclops meter run and stockpiled it for removal. I also found a load of clean top-soil on site. The contaminated area had been scraped down approximately 12 inches.

1797249445 2015-09-01 Non Well No Violations I received a phone call from Dean Limbacher, production superintendent for NGO, to report a crude oil release as a result of a vehicle accident. He reported 2.5 to 3 barrels of oil down on the surface in a hay field. I met him at the Pew-Cyclops meter run, located on the east side of CR 10, 2.7 miles north of SR 36 and found that a 5 barrel drip tank had been struck, spilling its contents onto the ground. Scott McCune, pumper for NGO, was also on location. He claimed that the tank was half full at the time of the accident. They had the oil covered with peat moss. No oil had escaped the field. There were no waterways affected and no observed impact to local wildlife. I instructed Dean and Scott to excavate the contaminated soil, haul it away and remediate the site with fresh top soil, seed and straw.
