0992409518 2019-04-11 Production Wells Violations Noted On the day of my inspection I found that the well was equipped with a plunger lift. The flow lines were connected, and the valves were open. I found no leaks around the wellhead. There was also a drip, regulator and house gas meter at the wellhead. I met the property owners at the site. They said that the well makes enough gas to heat the home, which is empty right now. The production chart showed steady gas production. The production equipment consisted of two 100 barrel steel tanks and two horizontal separators in an earthen dike. The chart in one of the separators was labeled Stewart #1, although there was no indication of the Stewart on the identification signs. Correct, legible identification was posted for the DeMoss #1. The soil around both tanks was saturated with crude oil. The bottom edge of both tanks was corroded and covered with sludge. The 2 inch diameter steel line running from the ground to one of the tanks was also corroded and covered with sludge. Both tanks were empty, and I could not determine the exact location of the leak. The chart was dated 3/20/19, so someone has visited this site within the past 30 days. The oil impacted soil was covered with leaves and debris, indicating that it has been this way for some time. I also found crude oil running out the side of the cabinet on the separator that was labeled Stewart #1. There was an open line inside of the cabinet. The crude oil was bubbling out of the top of the line where there was an open threaded fitting. I conducted an inspection on the Stewart #1 and spoke to the well owner. It was sold as a domestic well in December 2017 and is no longer plumbed to the separator or tanks at this location. The well owner must identify the source of the leak around the tanks and make the necessary repair, or replacement of the defective equipment. 1195377460 2019-04-23 Production Wells No Violations On the day of my inspection I found that no work had been done to clean up the oily soil around the tanks.
1555084100 2019-04-12 Production Wells No Violations I called Chuck Woods with M&R investments yesterday after I issued the compliance notice for pollution and contamination. Chuck disputed their ownership of the separator that was leaking. He said that they didnt buy that well and the old production equipment still belonged to NGO. I called the Energy Cooperative to report the issue, and Don Wallace returned my call. He said that he wasnt sure about the ownership, but he would send a crew out to take care of the oil until we can determine who is responsible for removing the rest of the flow line and the separator. Derrick Buxton called me this morning to let me know they were going out to clean up the oil. When I arrived at the location I found that Buxton had already been there. The oil was all cleaned off of the front of the separator and inside of the cabinet. The oil t hat was puddles in the gravel in front of the separator was also cleaned up. They had a rag stuffed in the top of the open line. I tried to call Derrick to ask if they were going to remove the line. He did not answered so I left a message.

1964171228 2019-05-23 Production Wells No Violations On the day of my inspection I found that nothing had been done to take care of the oily soil in the dike around the tanks. I called Dale Smith and he said that his roustabout was on vacation until after Memorial Day, but he would send him out as soon as he is back to work.
