0299265868 2016-08-03 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a natural gas powered pumping unit. The engine was mounted, but the drive belts were not connected to the crank case. The flow lines were connected. The tubing valve was open and the casing valves were closed. There was a 3 foot diameter area of oily sludge and stained soil around the wellhead. Correct, legible identification was posted at the wellhead. The tank battery consisted of one 210 barrel steel tank and one horizontal separator. The gauge on the separator showed 100 psi. There were two drip bottles and house gas meters at the separator. The valves were open to the Meade property; the valves were closed to the Gibson property. There was a 55 gallon steel drum next to the drips that was split open at the bottom and leaking oil onto the ground. The oil impacted area was 1 x 5 running down slope toward the tank. There was no dike around the production equipment. The tank battery is shared with six other wells on the Stuller lease. Correct, legible identification was posted on the separator. I called Chuck Donley of TLC Oil. He claimed that he would be out to get the oil cleaned up and to remove the drum at the tank battery. He also claimed that they are waiting on Maram Energy to come out and set a new tank battery for the wells before putting them into production. Chuck said that the bulk of the lease is too far from the existing tank and they wanted to set tanks closer.
1042051285 2016-03-09 Production Wells No Violations On the day of my inspection I found that the service rig had been moved out. The well head was put back together and the valves were closed. The belts were still off of the engine. I found no materials or debris left behind. The site had been graded, but it will require seeding to re-establish vegetative cover.
1206246753 2016-03-01 Production Wells No Violations I received a call from Carrie Moore claiming that she had sold the wells on the Stuller lease to TLC Oil and Gas, owned by Ted Morris, Les Donley and Chuck Donley. She was concerned that they were servicing the wells and leaving them in a state of disrepair although they had never taken the proper steps to get registered and bonded with the State of Ohio. On the day of my inspection I found that the well was being serviced. Maram Energy was rigged up on the well, but no workers were present during the inspection. The tubing was in the hole and the rods were lying on the rack. The pumping unit was disconnected and the belts were off of the engine. Identification was posted at the wellhead, although it still reflected KMFAD as the owner. I found no spills around the work site. The tank battery consisted of one 210 barrel steel tank and one horizontal separator. The gauge on the separator showed 80 psi. There were two house gas meter connections and drip bottles at the separator. There was no containment dike around the production equipment. The well owner and contact number had been changed on the identification sign at the tank battery to reflect TLC as the well owner. I advised Carrie Moore that it was her responsibility as the transferor to submit the change of ownership form 7 within 30 days of the date of transfer per ORC 1509.31. I contacted Cynthia Marshall at DOGRM Central Office and found that she had no record of TLC Oil and Gas applying for an operator number. I contacted Ted Morris to advise him that they must cease all activity until they get registered and bonded with the State of Ohio.
1883610743 2016-09-06 Production Wells No Violations I received a phone call from Chuck Donley of TLC Oil to notify me that he had addressed all of the issues with the wells on this lease. On the day of my inspection I found that Les Donley was at the well to operate the pumping unit. The area around the wellhead head been cleaned up. There was a 100 barrel steel tank in an earthen dike back near the wellhead. Les said that they were setting tanks at the three back wells because the main tank battery was over a mile from the wells and they were concerned with leaks when transporting oil that distance through an old flow line. Les said that they would get ID signs posted on the tanks as soon as possible. There were no more leaks at the wellhead, and the contamination had been removed. This violation is resolved.
1937721492 2013-10-09 Production Wells No Violations On the day of my inspection I found that the well head was equipped with a pumping unit. The motor was hooked up and the valve was open. In its current state the well is capable of production. The tank battery consisted of one 210 barrel stock tank and one separator. Proper identification was posted and there were no leaks or spills. This well shares a tank battery with permits 2444, 2447, 2448, 2446, 2591 and 2697.
