0422759162 2015-11-25 Administrative Inspection No Violations I contacted Knox Co. inspector Mark McCombs to cover plugging operations in my absence. He reported that Randy Moran was planning to top the well off with pellet gel. I called Moran and told him that I did not approve the use of pellet gel for plugging. I told him that he must finish plugging the well with fire clay from an approved source, or cement.
0536702289 2016-10-12 Final Restoration No Violations On the day of my inspection I escorted surveyor Tom Smith to the plugged well site in order to mark the location with a flagged stake at the final recorded GPS coordinates. I found that the vegetation was well established and there were no signs of erosion. There was no evidence that the plugged well was leaking.
0542379644 2016-01-19 Plug / Plug Back No Violations I met with Randy Moran at his shop to deliver a written plug report for the well. I filled out all portions of the plug report that I personally witnessed. I went over the report with Randy to be sure that he clearly understood what information needed to be added and the requirement of having the report notarized prior to returning it to me.
1059580217 2015-11-24 Plug / Plug Back No Violations On the day of my inspection Randy Moran pulled the 8.625: surface casing from the well. After pulling the casing they washed 13 tons of clay down the well. The fluid level was still down approximately 150 to 100 feet when they ran out of clay.
1447858461 2015-11-18 Plug / Plug Back No Violations On the day of my inspection Randy Moran finished pulling the 3 casing from the well. He attempted to set a pre-cast cement plug on top of the 3 casing at 2150. The plug sat down at 1274. Moran made a good faith effort to push the plug further down the hole, but it would not go. When he pulled the tools out the end was plugged up with bits of shale, giving some indication that the hole had caved in while pulling the production casing. The tools were also covered in crude oil. The fluid level was down approximately 400. We agreed to bail the oil out of the hole before attempting to pull the 8 casing.
1448039398 2015-11-19 Plug / Plug Back No Violations On the day of my inspection Randy Moran welded a collar to the top of the 8.625 casing and cut it free from the conductor. He then bailed the well to remove the crude oil. After the oil was bailed out he set a brush and stone bridge at 1013.
1448986904 2015-11-23 Plug / Plug Back No Violations On the day of my inspection Randy Moran washed 5 tons of clay down the well to get a plug across the Berea prior to attempting to pull the surface casing.
1482588974 2016-06-08 Final Restoration No Violations On the day of my inspection I found that the production equipment had been removed and the tank battery site had been graded out. The well site had also been graded out and both areas had been seeded and covered in straw. There were grasses growing on the site, but the well was located in an ATV park and the restored area is subject to frequent vehicle traffic. The road that was once used to access the well is now being utilized as part of the ATV trail system. I found no signs of erosion around the area. I stopped and spoke with the park owner, who claimed that she was satisfied with the restoration. This site passes final restoration.
1533459745 2015-11-17 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran had 1.5 tubing set at 3134 inside of 3 casing. Cemenco pumped a 25 sack cement bottom plug. The well circulated during displacement. Cemenco displaced the cement to 400 off bottom. Moran pulled the tubing and Jet Wireline shot the 3 casing off at 2150. Moran started pulling the 3 casing from the well.
1629398809 2016-01-27 Plug / Plug Back No Violations I received a notarized copy of the completed plug report from Randy Moran. He documented that he topped the well off with 3 to 4 tons of clay allegedly left over from a prior job, and he provided a clay ticket. He also admitted in writing to orange-peeling the top of the conductor. I verbally warned J.R. Moran that they are not to orange-peel. They are required to cap the casing with a steel plate. This is not the first time that the Morans have been warned about the use of a steel plate to cap the well.
1667163861 2015-12-01 Administrative Inspection No Violations I contacted Randy Moran in regards to how he finished the plug job. He claimed that he used 3 tons of clay that he had left over from a prior plug job. I told Randy that he would be responsible for submitting a notarized copy of a partial plug report for the portion that was not witnessed by an inspector.
1825196529 2015-11-30 Plug / Plug Back No Violations On the day of my inspection I found that the service rig had been moved off of the location. The pits had been closed and the well had been covered up and the site was graded out. I spoke with the campground attendant, who claimed that Randy came out on Thanksgiving day to finish the plug job.
2072921848 2015-09-09 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a pumping unit. The motor was intact, the valves were closed and the polished rod was dry. The pumping unit was overgrown with small trees. The tank battery consisted of two 100 barrel tanks and one 50 barrel tank that had two holes in the side. There was no identification posted for the well. The production reports for this well show 0 barrels oil and 0 mcf gas from 1988 to 2003. There have been no production reports filed since 2003. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
