0048495330 2015-02-11 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The motor was intact, the electric was hooked up and the valves were open. The tank battery consisted of one 100 barrel tank, one separator and an earthen dike. Identification was posted and I found no leaks or spills. This well has not met the minimum production requirements since 2007. I contacted Carrie Moore to inform her that they need to make a minimum of 100 mcf gas or 15 barrels of oil.
0254913160 2019-04-22 Production Wells Violations Noted I received notification this morning that the tank was overflowing. A turkey hunter noticed the tank and reported it to the Woodbury Wildlife Area office. The area manager contacted DOGRM to report the issue. When I spoke with him he said that he was turning the power off to the well because it was pumping and oil was flowing from the top hatch on the tank. I called Jason Blocker with the emergency response team and I called Carrie Moore, the well owner. Carrie said that she went to the site and she was coming back with materials to start cleaning the area up. Jason said that there was 1 to 1.5 barrels of oil on the ground inside of the dike, and nothing had escaped containment. When I arrived at the site I found the oil on the side of the tank had not been cleaned off and nothing had been put down to absorb the oil in the dike. There was still a steady stream of oil flowing down the side of the tank. I estimated there was around 1.5 to 2 barrels of oil in the dike. It was not leaching through the dike wall and it was not near the top of the dike wall yet. The Coshocton Co. wildlife officer Jarrod Allison and the wildlife area manager Andy Herschner both met me at the site. I walked to the wellhead and found that the tubing valve was still open. I called Carrie Moore and told her to meet me at the site right away. She showed up and shut the valve at the wellhead and at the separator. She also called her water truck driver to have the tank emptied today. Carrie said that she would be back to start cleaning the side of the tank. I will follow up tomorrow.
0314556922 2017-08-17 Production Wells No Violations On the day of my inspection I found that the well was still idle. The motor was still off of the pumping unit and the valves were closed. I called pumper Jim Dunfee to ask about the status. He said that he didnt have an answer and he couldnt predict when the well would be back in production.
0475783457 2019-05-09 Preliminary Restoration No Violations On the day of my inspection I found that Carrie Moore had put a bridge across the stream to get into the well site. She still had a layer of oil soaked peat in the dike that needed to be removed. I also found the she had a load line ran from the tank to the stream crossing for the oil haulers to hook up to since they cant drive their trucks back to the tank site. There was no lock on the tank valve and there was not a plug in the end of the line. I called Carrie to let her know that she must have a lock or a bull plug to prevent an accidental release through that line into the stream. She did not answer her phone so I had to leave a voice message.
0600097692 2018-06-01 Production Wells No Violations On the day of my inspection I found that the well and production equipment was not accessible by vehicle. There was a tree down across the access road entrance and there was a culvert under the access road that was washed out to the point where the road was approximately 36 inches wide. The tank and separator were in a state of abandonment and completely overgrown with vegetation. What remained of the access road back to the wellhead was in a state of being overtaken by vegetative succession. The pumping unit was also in a state of abandonment. The electric pole was falling over and there was no motor on the unit. The lines were connected at the wellhead and the tubing valve was open, but the wellhead was completely overgrown with shrubs. This well was required to be plugged and abandoned by 6/7/17. Nothing has been done to put this well back into production, and there has been no application submitted for a permit to plug this well.
0940969385 2019-05-02 Production Wells No Violations On the day of my inspection I found that the well was pumping and I could hear fluid draining into the tank, which was 3/4 full. Carrie had her identification written on the side of the tank. The compliance notice that was issued in 2016 for non-production and no identification is resolved. The compliance notice that was issued in 2019 for pollution and contamination is still unresolved. Carrie did not have a bridge constructed to get across the creek into the well site, and she still had not removed the oily peat and soil from inside of the dike. I called Carrie to ask what she was doing but she did not answer. I left a voice message asking her to call me back.

1486408892 2017-02-06 Production Wells No Violations I had contacted Carrie Moore about an unrelated well and she questioned me about the compliance notice for this well. She claimed that everything in the report was false and she was disputing the claim that the well was idle. I explained the circumstances that led me to conducting an inspection on the well, and the conditions that led me to issue the compliance notice. She alleged that the well was capable of production, and she had pulled the motor off for repair just 5 days prior to my inspection. She also claimed that the Ridenbaughs were getting house gas off of the well. I went out to the well and found that the valve from the separator to the house gas meter had been partially opened. There was still no motor on the pumping unit. The tubing valve was open and one of the casing valves was partially opened. There was a steady gas leak from the 2.875 inch hammer ring/tubing packing. I did not hear any fluid bubbling in the top of the tubing or casing. The well is still idle and there have not been any production reports filed for 2015.
1519052378 2019-05-20 Production Wells No Violations On the day of my inspection I found that most of the oily peat had been removed from the dike. Carrie had put some gravel down in the dike. The soil under the gravel was clean, but there was still some oily peat up on the inside of the dike walls. I called Carrie Moore to ask that she get the rest of the peat out of the dike. I also reminded her of the lock for her tank valve. She said that she just ordered locks and would get it put on as soon as they arrive.
1541616290 2018-11-07 Production Wells No Violations On the day of my inspection I found that there were some changes at the well site. The downed tree had been removed from across the access road entrance. The overgrown vegetation had been trimmed around the production equipment. There was an electric motor mounted on the pumping unit; however, there were no drive belts on the pulleys. The polished rod was dry, and all of the tubing and casing valves were closed. There was not a gauge in place to determine if there was any gas pressure at the wellhead. The valves at the separator were closed and the tank was empty. The domestic gas meter read 0500, but neither of the dials were turning. KMFAD Oil and Gas took ownership of this well in 2014 and correct identification has still not been posted at the wellhead or tank. A compliance notice was issued in 2016, requiring this well to be plugged by 6/7/17. There are no records of a plugging permit ever being issued for this well.
1556200554 2019-04-25 Production Wells No Violations On the day of my inspection I found that there was still a layer of crude oil in the dike. The bulk of the oil had been vacuumed up two days ago, but now the oil that was left behind is starting to solidify with the drop in temperature. I spoke to Carrie Moore yesterday and she claimed that she would have all the oil out of the dike by this morning. I called her again and she said that she was on her way out here. Carrie did show up to work on the oil clean up. She was trying to pump the oil into 5 gallon buckets with a sump pump when I left the site. I told Carrie that she must call me to inspect the dike before she dumps any gravel in there. She had a bail of peat and she claimed that she was going to spread it on the oil once she had pumped all she could. She claimed that she would then shovel the oil and peat out of the dike. I told her I would be back out tomorrow to check on it.
1556285684 2019-04-26 Production Wells No Violations On the day of my inspection I found that the bulk of the standing crude oil had been pumped out of the dike. The remaining oil had been covered with peat moss. I called Carrie Moore and discussed her next step in the clean up. She needs to get a solid bridge to cross the creek to get equipment into the site. Then she needs to shovel the peat and oily soil out of the dike until she gets down to clean soil. Carrie said that she planned to get the bridge in today. I will check back on Monday. The pumping unti was in operation at the time of my inspection. I also know that Carrie Moore has shipped two tanks of oil from this well. The compliance notice from 12/1/2016 can be resolved as soon as Carrie gets proper identification posted.
1556547689 2019-04-29 Production Wells No Violations On the day of my inspection I found that there had been no progress since my last inspection on Friday morning. The oily peat moss was still in the dike. On Friday Carrie said that she was going to get a new bridge in place over the weekend to cross the creek with equipment to finish the clean-up. Nothing had been done. I called Carrie but she did not answer. I left a voice message. The well was pumping during my inspection, and the tank was half full.
1558108310 2019-05-17 Production Wells No Violations On the day of my inspection I found that nothing more had been done at this site. The oily peat was still in the dike and a lock had not been installed on the tank valve. I tried to call Carrie Moore but her phone went straight to voicemail.
1559915497 2019-06-07 Production Wells No Violations On the day of my inspection I found that the rest of the oily peat had been removed from the inside of the dike. The compliance notice that was issued in response to the tank overflowing is resolved. Carrie Moore was still working on getting a proper bridge to get a vehicle back to the tanks, so she still had a load line running from the tank to the creek crossing. The line was capped but there she still did not have a lock on the load valve. I called her to remind her that she needs a lock. She said that she had her locks and she would be out this afternoon to get in on.
1594009892 2019-04-17 Production Wells No Violations On the day of my inspection I found that there was a 5 gallon bucket of oil laying on its side under a valve on a steel load line at the access road entrance. The oil had spilled out onto the ground. The whole area was wet and there were multiple wash-outs. The soil around the culvert had completely eroded away. I did not find any signs that the oil from the bucket had made it to any of the erosion ditches or the main creek. There was a rubber load line in the middle of the access road that was connected to the tank valve. The tank was full of fluid. The access road was washed out on one side. The pumping unit had been equipped with an electric motor. The drive belts and electric line were intact, and the breaker switch was in the ON position. The valves were open at the wellhead. There was a plastic tub of water sitting next to the wellhead. I sent Carrie Moore a photo of the oil and requested that she get it cleaned up today. She called me to say that she was on her way out. She also claimed that this well is back in production and she has already shipped 10 barrels of oil. She said that she would be shipping the full tank this week. I asked that she provide a run ticket to show proof that the well is producing oil. Carrie sent me photos later in the day to show that she had the oil cleaned up that was in the grass around the load line.
1893826949 2019-05-01 Production Wells No Violations On the day of my inspection I found that Carrie Moore was at the site working on a new bridge to cross the creek with equipment. She also spread another bail of peat on the remaining oil in the dike. Carrie claimed that she would have the clean-up done by the end of the day, and she would get correct identification on the tank.

There was a small tree that had fallen over the access road and there were no signs of any recent traffic back to the wellhead. I tried to call Carrie Moore and she did not answer.
