0244453089 2015-09-22 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler was rigged up to plug the well. He was pulling rods and tubing.

0879732435 2013-12-18 Production Wells No Violations On the day of my inspection I found that the well head was equipped with a pumping unit. The tank battery consisted of one 100 barrel stock tank, one 30 barrel brine tank and an earthen dike. Proper identification was posted and there were no leaks or spills. No violations at this time.
1166623808 2015-09-23 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler washed 2 tons of clay down the 5.5 casing. When he tried to rip the 5.5 casing the tools got hung at 1100. He came out, removed the knife and went back in. The tools still would not go past the tight spot at 1100. He washed another three tons of clay down the 5.5 casing for a 5 ton bottom plug. Wheeler proposed going in with a mandrill to try to ream the casing in order to allow the ripping tools to get deeper.
1427992606 2015-04-02 Production Wells Violations Noted On the day of my inspection I found that there was a swath of paraffin on the ground, approximately 3 ft. wide and 20 ft. long, where a service rig had laid the rods down to service the well. I met the pumper at the well and gave him a verbal warning that they need to get it cleaned up as soon as possible.
1443195943 2015-09-25 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler continued ripping the 5.5 casing at 2370 until he finally got it to part. He started to pull and but had to shut down for the weekend due to an equipment breakage.

He set a pre-cast cement plug on top of the casing where it was ripped down hole at 2370 and washed another 5 tons of clay down the well to get a plug at the top of the Big Lime.
1443710674 2015-10-01 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler set a brush and stone bridge at 940 and washed 4 tons of clay down the well to set a plug across the Berea. He pulled all of the 8.625 casing from the well, set a pre-cast cement plug on the 8 seat and tagged it at 417. Wheeler washed 6 tons of clay down the well. He ran out of clay and had to shut down for the day.
1443794811 2015-10-02 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler washed another 5 tons of clay down the well, filling it to surface. They cut the casing off 36 below grade and capped the well with a steel plate inscribed with the permit number.
1444333370 2015-09-24 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler reamed the 5.5 casing at 1100 with a mandrill tool. He was able to get his ripping tools past the tight spot and found a telegraph at 2370. He started ripping the casing at 2370, but he did not get it free.

2071837016 2016-06-09 Final Restoration No Violations On the day of my inspection I found that the well site had been graded out and was growing with well established tall grasses. The tank battery is still in place and in use for permits 6329 and 6341. The gravel drive is still in use to access the tank battery. There is also a gas riser and drip bottle at the site that is still in use for the existing wells. This site passes final restoration.
