0134757554 2019-04-08 Production Wells No Violations On the day of my inspection I found accompanied DOGRM surveyor Tom Smith back to the wellhead to record the well location. I found that the gas leak had not been corrected and the oily soil around the wellhead had not been cleaned up yet. I called Louis Yoder with Jerry Moore Inc. to ask what they were doing. He said that he sent someone out there a couple weeks ago but they must have gone to the wrong well, because they reported back that there were no leaks. Louis took my number and said that he would have his pumper call me to meet at the well. He also said that he plans to have the well plugged within the next month. The pumper called me and to get directions to the well. He said that he would be out tomorrow.
0581213754 2019-05-13 Final Restoration No Violations On the day of my inspection I found that all of the wellhead materials and production equipment had been removed from the site. The gravel was still in place on the access road and well pad. The soil was rough graded and there were several areas of standing water around the site. When I walked the site I found a 5x7 foot rectangular area where gas was bubbling out of the soil and standing rain water. The location of the bubbling was in the general area where the production equipment had been located. I found no signs of leaking around the actual well. I called Lewis Yoder with Jerry Moore Inc. to report the issue. He said that he did not have any active lines in the area, but he would call someone to come out to investigate. I also called and left a message for Chris Kimble with Red Hill Development, because the production equipment for their Bluck #2 well, which was plugged in 2017, was also located in the general area of the leaking gas. Finally, I called Chuck Woods with M&R Investments because they have several producing wells within a mile of the location, in all directions. Chuck said that he would send one of their former NGO employees out who has an extensive knowledge of the existing sales lines. I met the line tech for M&R at the site. He consulted his maps and found no M&R lines through this site. He also had a map of Aspires lines, and we found that there was an Aspire line approximately 1000 feet south of this location. I called Chris Kimble back and he also had a line tech in route. Red Hills tech showed up and started digging where the gas was coming up through the ground. He found a line marker and he said that they did have a sales line going through this area, but he wasnt sure it came this far east. A second line tech from Red Hill showed up and they walked the line with a locator to determine whether or not it was theirs. They did determine that the leak was likely from their sales line. The two techs dug for a bit with shovels and found signs t

0790870701 2019-05-14 Non Well No Violations On the day of my inspection I found that Red Hill Developments sales line had been repaired and the leak was stopped. There was a large area of recently excavated soil and tracks where the leak was found yesterday.
1494775044 2014-03-24 Production Wells No Violations This well is equipped with 2.875 tubing. It is located next to the tank battery for Red Hill Development permit 6271. The records on file indicate that this well was sold to the landowner by Jerry Moore in 1984. The records indicate that the well is maintained by Red Hill and the landowner gets house gas. There are no current production records. I took updated GPS coordinates as per request from DOGRM Central Office. I called Red Hill and Jerry Moore Inc. to get some answers. I will have to wait for someone to get back to me.

1555945475 2019-04-22 Plug / Plug Back No Violations On the day of my inspection I found that Chipco was rigged up on the well, with 1.5 inch tubing ran to a depth of 4120 feet. They pumped a 15 sack bottom plug with class A cement in the 2.875 inch diameter casing. Chipco pulled the tubing and Appalachian Well Surveys came in to shoot the production casing off. Chipco has about 30 inches of stretch on the 2.875 inch casing. Appalachian tried to shoot the casing off at a depth of 2800 feet. The first shot was unsuccessful. Appalachian went back in for a second attempt. The casing still would not pull free. Chip Smith said that he calculated the free point at 2818 feet based on his stretch. He asked if they could come up 100 feet to 2700 to make a third attempt. I agreed with his request, and we both agreed to set a 300 foot plug at the top of the Big Lime. The third attempt was not successful either. Chipco tried to pump on the casing and it pressured up immediately, indicating that the shots did not even make a hole. Chip decided to shut down for the day and bring the shooters back tomorrow morning to make another attempt.
1556037824 2019-04-23 Plug / Plug Back No Violations On the day of my inspection I found that Appalachian Well Surveys had run a bond log first thing in the morning. The log showed that the 2.875 inch diameter casing was bonded solid up to 2605 feet. The log also showed bond at 2450 feet and 2360 feet. Appalachian attempted to shoot the casing at 2558 feet. Chipco pulled up to 70,000 pounds on the casing and it still would not come free. Appalachian came up to 2329 feet and shot a second time. The casing finally pulled free. Chipco pulled one joint and pumped a 300 foot cement isolation plug above the Big Lime. Chipco pulled up to 922 and pumped a 200 foot cement plug through t he Berea. I weighed a cement sample for the Berea plug, which was up to 14.9 pounds per gallon. They were within the 5% variance for class A cement. Chipco pulled up to 578 feet and pumped a 200 foot cement plug at the base of t he surface casing. They pulled up to 200 feet and pumped until cement was circulated to the surface. Circulation was established and maintained throughout all plugs set today. Once Chipco was rigged down we checked the cement level. It had only fallen three feet. They already had a steel plate made up with the permit number welded on. They were going to cap and bury the well today once they got all their equipment out. They also removed the separator and all other well materials.

1872450569 2017-07-27 Production Wells No Violations The status of this well came into question while I was conducting a final restoration inspection for permit 6271. Both wells were in the same field and I recalled speaking with Red Hill in 2014 when they told me that they swab and maintain the well for a landowner to use for domestic gas. Now that Red Hill has plugged their Bluck well I called to ask what the plans were. They said that they would have to get back to me on that, but they did confirm that the separator and house gas meter left on site was for this well. I called Jerry Moore Inc. to ask about the well. I told them that they need to follow through with the paperwork to have the well officially transferred to the land owner as a domestic well if that is what they wish; otherwise, they must produce the well. They also told me that they would get back to me.
