0005553038 2019-05-03 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had another 2.5 tons of clay delivered at the site. Freeman said that they filled the well to surface again last night when they brought the clay in. I dropped a clay clump down the well today and found that the fluid was back down to 100 to 120 feet. Freeman said that they put about half of the new clay down the well yesterday. They got about a half ton in today before the well filled to surface again. It was receding slowly when we left. Freeman said that they might be back out this evening to top it off again, and then they wont be back out until Tuesday next week.
0064818189 2018-09-10 Production Wells No Violations I was contacted on September 4th by Freeman Miller in response to the compliance notice to plug the well. Freeman said that he wanted to clean the well out in the spring in an attempt to produce it. The well has been idle since 2006. I initially contacted the Millers in July of 2017 to request that they produce the well or plug it. I received a landowner complaint and issued the compliance notice in January of 2018. They did not respond or act within the allotted 180 days. On the day of my inspection I found that there were no signs of any activity at the well. I called Freeman to notify him that I was denying his request for an extension to attempt to produce the well. I told him that it must be plugged. He request the contact information for the Southeast Regional Supervisor and said that he would get back to me with their plan.
0165632465 2019-05-20 Plug / Plug Back No Violations On the day of my inspection I found that the clay had not moved since my last inspection. Miller Oil had not been in yet to cut and cap the well. They still had their equipment at the site.
0225317306 2018-10-23 Production Wells No Violations On the day of my inspection I found that nothing had been done to equip the well for production and nothing had been done to prepare the site for plugging the well. A plugging permit has not been issued for the well. I spoke to the landowner Brian Darr before visiting this site and Brain said that he wanted to file an official complaint in the DOGRM Complaint Log. I entered the complaint under complaint number 5993. I called James Miller to ask what he was doing with the well. He claimed that they were going to plug it, and he claimed that he would get his permit application in today.
0245004259 2019-03-14 Production Wells No Violations On the day of my inspection I found that Miller Oil was moving in and rigging up to plug the well. The brush around the wellhead had been cleared away. Freeman Miller said that they hoped to have the tubing out by the end of the day, and they plan to get the clay next week. I reminded Freeman to give me prior notification when they get the clay and start the job. I tried to call the landowner Brian Darr to let him know they were moving in.
0598977766 2018-01-26 Production Wells Violations Noted On the day of my inspection I found that the well was shut in with a valve on top of the tubing. The wellhead was surrounded by vegetation. There was no identification posted and I found no production tanks for the well. I contacted the landowner Brian Darr to ask if this well has ever been, or is currently being used for domestic gas. Brian said that this well used to make domestic gas for the home on the property, but the home no longer uses gas from this well. Brian said that the domestic gas for the home now comes from the Gress-Wilkins #1, permit 7005 which was put into production in 2006. The only annual production reports filed since 1990 are for 2014 and 2016, which show the exact same production reported for permit 7147. The well is not being produced commercially and it is not providing domestic gas. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
0776854848 2019-03-20 Plug / Plug Back No Violations On the day of my inspection I found that no one was working at the well site. Miller oil did not have any clay delivered yet and there were no signs of any activity since they got the tubing pulled.
0871427716 2019-04-11 Plug / Plug Back No Violations On the day of my inspection I found that the fluid level had fallen back down to approximately 100 feet. Miller Oil was not working at the site. I spoke to Freeman and he said that they did not have all of the materials they need to start pulling the surface casing. He said that it would take most of the day to gather what they need. I will check back tomorrow.

1125618244 2019-02-25 Production Wells No Violations On the day of my inspection I found that nothing had been done to prep the site to move in. The brush had not been cleared around the wellhead and there were no changes in the wellhead equipment. I called Freeman Miller to ask what their plans were. He said that the thought they had two years, according to the date on their permit. I informed Mr. Miller that the well is under a compliance notice, which was generated by a landowner complaint, and it was supposed to be plugged by 7/26/18. He said that their rig doesnt start or run well in the cold weather and he wants to wait until it warms up. I got Freeman to agree on a tentative move in date of no later than the second week of April, 2019. I made sure he knew about the notification requirement, and he agreed to contact me when they are planning to move the rig in.
1151527058 2019-05-02 Plug / Plug Back No Violations On the day of my inspection I found that the fluid was still around 100 feet deep. There was still a little clay left in the pile. I called Freeman Miller to ask what they are doing. He said that he was getting ready to go get another two tons of clay from Belden Brick and he planned on coming out to the well site later in the day. I asked that he call me to let me know when he was going to be here.
1377951862 2018-07-18 Production Wells No Violations On the day of my inspection I found that the well was shut in and completely overgrown with thick vegetation. There were no signs that any work has been done to bring the well into production, and no identification has been posted. I found the well to be idle on 1/26/18 and issued compliance notice to A. Miller Oil, which allowed 30 days to reply with a plugging plan. No plugging plan was ever submitted. The compliance notice also requires that the well be plugged within 180 days. No plugging permit has been issued for this well. I called James Miller to ask what he was doing. He admitted that he forgot about it, but they were most likely going to plug. James said that he would get this permit application in and he would be in touch when they were going to move their rig in.
1464709759 2019-05-16 Plug / Plug Back No Violations On the day of my inspection I found that the clay was still at the same depth as it was when we left the site yesterday. Miller Oil used a total of 11 tons of clay to fill the well to surface. I contacted Freeman to let him know that they could cut the casing off and cap the well. I also reminded him that it needs to be cut off at least 36 inches below grade. The compliance notice for non-production is resolved.
1478135061 2019-01-03 Production Wells No Violations On the day of my inspection I found that Miller Oil had not made any progress with moving in to plug the well. I called James Miller and he claimed that they wanted to wait for the ground to freeze.
1480629605 2017-07-18 Production Wells No Violations On the day of my inspection I found that the well was equipped with 2.675 inch diameter tubing that was shut in with a valve. The surface casing was partially exposed, but the nipples were buried. There was a gas regulator and 1 inch diameter plastic line that was coming from underground. The wellhead was surrounded by vegetation. There was no identification posted and I found no production tanks near the well. There are no annual production reports for this well from 1990 to 2014, when A. Miller Oil reported 49 barrels of oil. There are no production reports after 2014. I called James Miller to ask about the status of the well. He said that it used to be a domestic gas well for the home on the property, but there is a newer well drilled on the property now and James wasnt sure if they still used gas off of this well. I told him that it needs to be put into production or plugged.
1544023158 2018-12-05 Production Wells No Violations On the day of my inspection I found that there were no changes at the well site. There were no signs of any site preparation to move in and plug the well. There was a plugging permit issued to A Miller Oil on 11/9/18. I called James Miller to ask what their plans were. He did not answer so I left a voice message.
1552660681 2019-03-15 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil was rigged up and in the process of pulling the tubing from the well. They had about half of the tubing out laying on the ground. I spoke with Freeman Miller to go over the specifics of the plug job. They plan to have all the tubing out today, and they should have their clay by the first of next week. Freeman said that they will be using casing jacks to remove the surface casing.
1552918919 2019-03-18 Plug / Plug Back No Violations On the day of my inspection I found that all of the tubing had been pulled from the well. The fluid level was down to approximately 150 feet. No one was working at the site. I called Freeman Miller to ask about their schedule, He did not answer so I left a voice message. The landowner Brian Darr called me to ask about the schedule for getting the well plugged, because he wants to put field tiles in through the site and he wants to make repairs to the access road.
1553276465 2019-03-22 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had a load of clay delivered to the site. They were not prepared to start running clay yet. When I dropped a rock to gauge the fluid depth I found that it was only down 150 feet. I spoke to James Miller to discuss our options with the current fluid level. James said that he would bail the hole dry before they start washing clay in. The current weather conditions are rain and wintery mix. James said that they want to wait until next week to start because of the weather.
1553627141 2019-03-26 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had moved their water pump and power tongs in, and had everything set up to start the job. No one was working at the site. I spoke to Freeman Miller about when they were planning to start. He said that it would likely be this week. I also spoke with Freeman about the fluid level and the need to bail t he hole down before adding clay. Freeman proposed adding clay and letting the water come up and drain into a pit. I opposed this suggestion and we discussed the need to get the fluid level down to allow the clay to get to the bottom without bridging off in the fluid column. I also discussed the specific site topography and the close proximity to a pond. I told Freeman that I want them to bail the hole down before they start running clay, and I want them to bring in a tank to bail into, rather than digging an earthen pit. Freeman agreed to my request and he said that it would take him a day or two to get the rest of his equipment together.

I spoke with Freeman Miller and he said that they still had to make a repair to their bailer before they could get started. He predicted that they would be able to start on Tuesday next week.

I called Freeman Miller and he said that they had to pump today, but they would be out tomorrow to get this job started.
1554905081 2019-04-09 Plug / Plug Back No Violations On the day of my inspection I met James and Freeman Miller at the site to start bailing the hole down in preparation for clay. The fluid level was approximately 150 feet down when they started. Freeman bailed between 25 and 30 barrels of water from the well and the fluid level stayed exactly the same. The water that was bailed from the well was clear and there was no oil. We suspected that there could be a hole in the surface casing at around 150 feet. The Millers started running clay. They washed close to 2 tons in and the fluid level still did not change. They will be back tomorrow to finish the bottom portion of the clay plug, and to start pulling the surface casing.
1554922308 2019-04-10 Plug / Plug Back No Violations On the day of my inspection I met Freeman Miller at the well site to continue with the bottom plug. When I arrived I dropped a clay clump down the well and found that the fluid level was still down approximately 150 feet. Freeman slowly washed another 3 tons of clay in the well over the course of the day. The well never filled to surface, but t he fluid level did come up to around 50 feet by the time they were finished. They got a total of 4.5 tons on the bottom. They will allow the clay to settle over night before trying to pull the surface casing. Freeman said that he would be back tomorrow morning to start trying to remove the 7 inch diameter casing.
1555355526 2019-04-15 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had a 8.625 inch diameter sub screwed onto the surface casing. They also had there elevators and slips in place. They were not working at the site. When I spoke to Freeman Miller he said that they would be out on Thursday, 4/18/19 to start pulling the surface casing. The sub was over 10 feet long and I had no way of checking the fluid level.
1556214850 2019-04-25 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had surface casing out of the well, hanging in there elevators. Freeman said that they got the casing jack engine back yesterday evening and they came out to try to get the casing free. He said that it did not jump when it gave loose. They pulled the casing that was free out of the well and measured it at 66.4 feet. The last joint came loose at the threads. The casing was pitted and full of holes. They set their equipment up to start running clay. I dropped a clay clump to check the fluid depth. It was down to approximately 100 feet. The Millers got about 2 tons of clay washed in before the well filled to surface. They said that they would leave it open overnight and check it tomorrow.
1556310894 2019-04-26 Plug / Plug Back No Violations On the day of my inspection I found that most of the clay pile was gone. There was maybe 1 to 1.5 tons left. I dropped a clay clump in the well to estimate the fluid level, which was back down to approximately 100 feet. The Millers had not made it out to wash any more clay down the well today. Freeman Miller contacted me later Friday night to report that they had been out to fill the well up again, and they moved their rig out. I will follow up on Monday.
1557758201 2019-05-13 Plug / Plug Back No Violations Freeman Miller contacted me Friday evening to let me know that they were back out to top the well off with more clay. He said that it did not take long to fill up. On the day of my inspection I found that the fluid level was within 25 to 30 feet from the surface. There was still approximately a half ton of clay left under the tarp. I contacted Freeman Miller to make arrangements to meet them at the well site to witness the next top-off session.
1558625926 2019-05-23 Plug / Plug Back No Violations On the day of my inspection I found that the well had been capped and covered with soil. I received photos from Freeman Miller showing that they cut the casing off at 36 inches below grade, and showing that they put the steel plate on top of the casing before they buried it. The well site had been graded out to the contour of the surrounding landscape. The only materials left at the site were one 50 barrel steel tank and one bulldozer.
1758903404 2019-04-29 Plug / Plug Back No Violations On the day of my inspection I found that Miller Oil had moved their service rig off the location. The bulldozer and back-hoe were still there. There was a small amount of clay left in the pile. I dropped a clump and found that the fluid level was still around 100 feet. Freeman Miller said that they had to pump wells today, so they probably would not be back out until Tuesday or Wednesday.
1761108611 2019-04-12 Plug / Plug Back No Violations O n the day of my inspection I found that no one was working at the site. I spoke to Freeman Miller to ask what his plan was. He said that they got all of their materials together for pulling the surface casing and he would like to get everything moved in and rigged up today. He said that he has meetings for the next few business days, so they would not be back to work at the plug job until Thursday next week. The fluid level was at the same depth as it was yesterday, around 100 feet.

Freeman and James Miller filled the well to surface with clay again. It did not take as long to fill up this time. I asked them to leave the well open overnight and I would check it in the morning. They brought the steel plate out to cap the well. They also had a mini-hoe and troches on site to cut the casing off when it is determined to be full.

I called Freeman Miller and he said that they were out to fill it up on Tuesday, and he planned to get out there again tonight. Freeman said that they have not bought any more clay since the last load.
