1188432871 | 2016-09-27 | Production Wells | No Violations | I received a phone call from UIC inspector Dave Ball to inform me that he had received a report from Devco of an oil spill in Coshocton County on SR 715. When I arrived at the location I found that the Devco driver was still on site loading oil from the storage tanks. The driver claimed that he was on top of the tanks while the truck was taking on the load. He said that the hose connector came apart while he was loading, allowing oil to drain from the truck and from the tanks. When he finished with his load ticket calculations, he said that he lost approximately 35 barrels of crude oil. The tanks are located uphill due to flood plain requirements. The load valve is located at the bottom of the hill near the McGinnis 5 wellhead. There is a blacktop drive from SR 715 to the well and the load valve. The oil traveled down the drive, out onto SR 715 and into the roadside ditch. There were no culverts or waterways in the area. The oil traveled approximately 300 feet west in the ditch. There was a load of sand piled in the ditch acting as a barrier to prevent the oil from traveling any further. There was also a load of sand dumped at the access drive entrance to prevent any more oil from entering the roadway. Dugan Oil and Gas pumper Kevin Slaughter was on location doing housekeeping on the lease road, and he said that the sand was delivered by a township trustee that happened to notice the spill while driving by. I also met with Jeff Kreager from Energy Services Corporation, the contactor for Devco. Jeff had a water truck, back-hoe and dump truck in route to start on the clean-up. Guy Dugan also showed up to assess the situation. The crew started at the road and was working their way back. I told Jeff to suck up as much oil as he could, then the remaining oil and impacted soil in the ditch and along the lease road would have to be excavated and hauled away. |

The standing oil on the lease road had also been treated with the absorbent material. The grassy areas bordering the lease road that had been affected were scraped down to the top soil and covered in peat. The crew was starting on the roadside ditch. I told Rick to excavate and haul any oil impacted soil, gravel or vegetation. Rick said that they would have the load profiled before going to a landfill and he would have paperwork. |

There was no admission of prior knowledge of the issues, and there was no admission of purposeful intent to cause pollution or contamination. Guy did not dispute the discussed compliance issues, and agreed to be cooperative. We discussed the actions that need to be taken to resolve the issues. I informed Guy that he would be receiving a mailed compliance notice which would list the actions needed to bring the site into compliance. I also told him that he would have 30 days to complete the actions. Guy claimed that he would be out to get started on Monday, October 10 to start the clean up. |

I walked the area with Barry, and found the lines. None of the lines were in the path of the impacted soil. I told Barry that they still need to address that area. He proposed churning the soil up inside of the dike and treating it with lime and fertilizer or peat moss. I agreed to let them treat in place as there were no significant areas of heavy staining, and there were no areas of standing fluid. We also found a new leak at the flow lines up at the main tank battery. Barry said that he would get it fixed prior to working on the lower dike. The contaminated soil around the wellhead and under the gearbox had been cleaned up. Guy Dugan produced a manifest showing that 2.51 tons of material was delivered to the Kimble landfill. |
