0272571607 2014-07-24 Final Restoration No Violations On the day of my inspection I found that the site had been graded, seeded and strawed. There was minimal grass growth. This site does not pass restoration at this time.

0883027156 2013-11-20 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed approximately 6 tons of clay into the well. It filled to surface and they let it settle.
1384542083 2013-11-15 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed 5 tons of clay down the 5.5 casing. They ripped the casing at 1800. The 7 casing was coming out of the hole when they started to pull the 5.5 casing. Matheny backed off 3 joints of 7 and pulled it. He then pulled on the 5.5 and it came free. They pulled all of the 5.5 casing from the well.
1384793559 2013-11-18 Plug / Plug Back No Violations On the day of my inspection Gene Matheny ran back in with the three joints of 7 casing equipped with an 8 skirt. They were able to get screwed back onto the 7 down hole. Matheny tried to pull on the 7 casing with no success. They set a brush and stone bridge at 975 and ripped the 7 at 930. They pulled approximately 400 of 7 casing when they started to get hung.
1384873955 2013-11-19 Plug / Plug Back No Violations On the day of my inspection Gene Matheny pulled one joint and found that the casing was still pulling hard. They stopped pulling and washed 4.5 tons of clay into the well. After washing the clay in they were able to start pulling casing again. They pulled the remaining 7 casing from the well.
1385055448 2013-11-21 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed approximately 2 tons of clay into the well. It filled to surface and sat firm. They cut the casing off 30 below grade, and capped it with a steel plate.
1403182805 2014-06-16 Final Restoration No Violations There is no evidence of production equipment left behind and the land was properly restored. The location has met all 1509.072B restoration requirements.
1661624072 2014-09-29 Final Restoration No Violations On the day of my inspection I found that the site was covered in well established short grasses. I found no signs of erosion. The well site was in a public hunting area and the lease road entrance is being maintained for parking. The main portion of the lease road has been modified with soil mounds to prevent vehicle passage. This site passes final restoration.
