0010895056 2016-04-13 Field Inspected, Well Not Found No Violations This inspection was conducted in response to a request to confirm the status or existence of this well. The location coordinates stored in RBDMS plotted the well in an open agricultural field 335 feet east of TR 251, across from address 24759. There were no remains of an access road. I found a bare depression in the middle of the field that was 48 feet from the coordinates stored in RBDMS. The depression was muddy and heavily tracked by deer. I found water and crude oil seeping from the ground in the center of the depression. I also observed active bubbling from the origin point of the seeping fluid. The physical evidence indicates that the well was plugged and is currently leaking. I went to the home, but there was no answer. I left a note and a business card in the mailbox.
0417903591 2016-05-19 Orphan No Violations Request by Orphan Well Program Administrator Michele Miller to do a detailed SOW (scope of work and detailed plugging plan). This well shows up in the Divisions data base as PR (Producing). This date I accompanied DOGRM County Inspector Chris Grimm to the well location. Chris has started the basic documentation for this well in the orphan well program. There is no well casing visible at the surface. There is a slight depression with evidence of brine and oil coming to the surface. We used our metal detector and showed a strong reading of metal below ground surface. This well appears to have been plugged. The Division has no plug report on file. I took 8 additional photos. The Dittmar property has a hand dug water well with an above ground metal hand pump. GPS for this well is N40.30695, W081.65422. We stopped at the Hostetter residence and obtained a contact phone number from the daughter in law. She knew nothing of the well. The Hostetter property made up part of the unit drilling acreage for this well.
0507728879 2016-05-23 Administrative Inspection No Violations I called my supervisor Gene Chini of the Orphan Well Program and advised him of my conversation with Mr. Hostetter. Gene asked me to contact DOGRM Water Inspector Jake Glascock to help with sampling and ground water depths. This date Jake Glascock was contacted and will start an investigation.
0692660634 2016-05-20 Administrative Inspection No Violations Bill Hostetter returned my call. He told me the well never produced, and was plugged about 5 years after it was drilled. He thought it was plugged with fire clay by an outfit called Production Services. I asked him if he knew it appeared to be leaking. Mr. Hostetter told me no, but he had drilled two water wells behind his pig barn that had crude oil in them. Both wells were plugged and he drilled a third water well that was ok. The water wells were drilled for water for his pigs. I asked him why he never called the Division and reported this. He told me he did not know who to call. Mr. Hostetter drives truck in the day time and is available in the evenings at home 740/545-9528.
0971319166 2018-12-18 Orphan No Violations On the day of my inspection I met Orphan Well inspector Jeff Fry at the well sight to witness the casing being excavated. The crew used a mini hoe to excavate the soil around the casing, which was open. They welded an 8.625 inch diameter pup joint to the existing casing and ran in with a wire-line depth counter. The device made it down to 3168 feet. The crew capped the casing and fastened a line to a 275 gallon poly tank to allow the well to vent and catch any fluid. They also installed a temporary fence around the wellhead.

1116151590 2018-12-24 Orphan No Violations On the day of my inspection I found that the well had not blown any fluid into t he poly cube tank, and I found no detectable gas venting from the well. The plastic line was still in place from the well casing to the tank, and the temporary fencing was still up. There were no other materials or debris around the well site.
1118109293 2016-04-14 Orphan No Violations On the day of my inspection I found that there was still water and crude oil seeping from the ground in the bare, muddy depression. There was still active bubbling from the center of the fluid seepage. I conducted a field test of the water with a high range Quantab test strip. The scale maxed out, indicating a chloride level equal to or greater than 6627 parts per million. I found that salt crystals had formed around the area. The immediate impacted area measured 10 x 15, with a 48 trail extending down slope. There were no water courses near the impacted area. The field was still in corn stubble from a previous years harvest. There was an area approximately 100 feet in diameter around the suspected well where no corn stubble was found. I searched the property for a water well. I found a manual water pump near the home. I also found a sub-

1463676745 2016-05-19 Orphan No Violations On the day of my inspection I accompanied orphan well inspector Jeff Fry to the well site, where we took updated photographs and assessed the site for potential access routes to the well. Jeff also surveyed the suspected well site with a metal detector, which indicated the presence of metal around the leaking bare area. We attempted to make contact with the neighboring Hostettler farm in order to get any information that they may have on the history of the well.
1465832143 2016-06-13 Orphan No Violations On the day of my inspection I met with DOGRM Hydrogeologist Jake Glascock at the well site to collect a baseline water sample from the water well at the Dittmar home. Mr. Dittmar was not present at the time of our inspection. We found that the hand pump at the home was inoperable. We surveyed the outside of the home and multiple out buildings and found no external water source. Jake was able to contact Mr. Dittmar by phone. He said the only source of water was located in the home and he could not make it down today. The actual hand dug well is located in a cinder block pit that is filled with large pieces of concrete debris and inaccessible for sampling. Jake and I walked out to look at the site where the well was leaking. There were small pools of crude oil and brine water and the soil was heavily crusted with salt crystals. We met with the adjacent property owner Bill Hostettler, who claimed that the water well currently in use for his home was drilled prior to the oil and gas well and he had no issues with oil in his water. He said that he did try to drill two 60 water wells behind his hog barn that both had oil in them. Mr. Hostettler also claimed that the oil and gas well was plugged within three to four years of being drilled. He said the he remembered the plugging company shot the production casing off down hole and pulled it; they washed one dump truck load of clay down the well, and left the surface casing in place. Mr. Hostettler claimed that the well has been leaking since the late 1970s.
1470687751 2016-08-04 Orphan No Violations This well in RBDMS is listed as PR for producing. The landowner Charles Dittmar wants to make application to the Orphan Well Program. This inspection well site is a depression bare of vegetation with no visible casing at the surface and no visible or audible leakage. There is still a 4 wide and 15 long barren area void of vegetation, that travels down gradient of the well location. I talked to Mr. Dittmar by phone this date and changed the Right of Entry (ROE) access to GPS N40.30714, W-81.65414. This access is approximately 175 north west of the previous ROE. A new sketch was done showing utility lines , and the new access route. I took ten (10) additional photos.

1768532834 2019-03-22 Orphan No Violations Orphan well inspector Jeff Fry asked me to stop by this well periodically to check on the 250 gallon poly cube. On the day of my inspection I found that the orange fencing was starting to detach from the stakes. There were no leaks from the well casing, and there had been no significant fluid production into the cube. There were no vehicle tracks, or any signs of activity around the well site.
