0110026418 2013-02-07 Plug / Plug Back No Violations On the day of my inspection I found that the service rig and all related equipment had been removed from the site. The pit was closed and the soil was graded out to the approximate original contour. The lease road will remain in place at this site, as it is used to access the tank battery for permit 4780.
0175881751 2013-06-06 Final Restoration No Violations On the day of my inspection I found that the former well site was graded out and plowed for agricultural crops. The tank battery and access road are currently in use for permit 4780. There were no leaks and there were no oilfield related materials or debris left at the location. This site passes final restoration.
0264112941 2013-01-03 Plug / Plug Back No Violations On the day of my inspection Timco Wireline Services ran in with a stem to 130 to make sure they could get their guns past the rotted 7 casing top at 86. They repeatedly got hung up when trying to run in with the shots, and eventually lost the weight from the bottom of the gun. The decision was made to run back in with the sewer camera before any further attempts at getting into the 7 casing.
1022918820 2012-12-31 Plug / Plug Back No Violations On the day of my inspection Collins and Rice Well Service pulled 86 of 7 casing from the well. They ran 4.5 casing equipped with an 8 swedge to 85 to hang just above the rotted off 7 casing. Then Billy Patten went in with a wire line sewer camera to determine the extent of the damage at the top of the 7 casing. The decision was made to run in to shoot holes in the 7 casing, shooting 10 holes every 100 from 1000 to 200.

1357672288 2013-01-07 Plug / Plug Back No Violations On the day of my inspection Patten Drilling ran a sewer camera down to the top of the 7 casing. The image revealed jagged burrs of metal folded over blocking the opening at the top of the 7 casing. Collins & Rice Well Service ran in with 1.5 tubing coupled with a 3 string mill back down to 1.5. The set up eventually penetrated the burrs and, after working the mill in the tight spot, the top of the 7 casing was opened to approximately 3.
1357919425 2013-01-10 Plug / Plug Back No Violations On the day of my inspection Collins & Rice Well Service had 1.5 tubing set at 965 inside of the 7 casing. Formation Cementing pumped class A cement with 3% gel through the 1.5 tubing until the well was filled to surface. All the tubing was pulled from the well and the 85 of 4.5 casing was pulled from the well. At that point the cement had fallen approximately 50. Formation then filled the well from surface. The cement top was at 10 and falling slowly. We will return on 01/14/2013 to tag the top of the cement plug and determine if further action is needed.

1359387363 2013-01-25 Administrative Inspection No Violations FOLLOW UP TO CA # 1164726813: I received a phone call from Billy Patten to notify me that the well had been topped off with clay. They were cutting the casing off 36 below grade and capping it with a steel plate. The pit was closed and the rig was moved off location.

1476376567 2016-10-12 Final Restoration No Violations On the day of my inspection I escorted surveyor Tom Smith to the plugged well site in order to mark the location with a flagged stake at the final recorded GPS coordinates. I found that the vegetation was well established and there were no signs of erosion. There was no evidence that the plugged well was leaking.
