0423181732 | 2017-08-09 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran had the rods out, and he was pulling the tubing from the well. They had a load of fresh water delivered. We discussed the need for precautions with regards to the well being located in an H2S township. |
0867013518 | 2017-08-10 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran finished pulling the tubing from the well. He then washed two tons of clay down the 4.5 inch diameter casing to start the bottom plug. They ran into problems when they tried to remove the wellhead components and ended up having to cut them off. By the time Phil got the casing prepped and the ripping tools together they decided to shut down for the day. |
0900229465 | 2017-09-08 | Final Restoration | No Violations | I visited the site with Tom Smith to survey the plugged well location. The well site had been graded out, and all the equipment and debris had been removed from the site. No seed or straw had been put down to restore vegetative cover to the site. |
1070835014 | 2017-08-15 | Plug / Plug Back | No Violations | On the day of my inspection Petroset pumped through the 4.5 inch casing to establish circulation. They pumped a 150 foot cement plug through the Berea. Phil pulled the 4.5 inch diameter casing up to 100 feet below the base of the surface casing. Petroset established circulation and pumped a 200 foot cement plug. Phil pulled the casing up to 200 feet and Petroset pumped until cement was circulated to surface. The plug fell 20 or 30 feet after they got the rest of the casing out. Phil said that he would top it off with clay in the morning. |
1502373135 | 2017-08-08 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was rigged up to plug the well, and he was pulling rods. They had an 11 ton load of clay delivered from Logan Clay. |

1502724178 | 2017-08-14 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was not at the site. I did notice that there were three joints of very deteriorated 8.625 inch diameter casing laying on the trailer. The clay trough was in the 4.5 inch casing and the clay pile appeared to be smaller. I called Phil to ask for an update. He said that he would not be out to the site until 13:00 today and he forgot to notify me. He said that the 8.625 inch casing was starting to come when he was working on the 4.5 inch casing, so he pulled what he could. We have had suspicions about the condition of the surface casing since the beginning of the job. He also said that he washed another 5 tons of clay down the 4.5 inch casing after he ripped it. Phil said that he could not get a telegraph until 1510 feet and now that the casing is parted he still cant get it to pull freely. I met back up with Phil later in the day and he did get the casing to pull. They pulled it up to 900 feet in order to set a cement plug through the Berea. |
1502975613 | 2017-08-17 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran had rigged down and moved out. The pit was closed and the site had been rough graded. I met Phil at the site and he said that the cement had fallen approximately 70 feet. Phil speculated that there was a water seam at that depth because that is approximately where the surface casing was rotted off. He said that they used to two tons of clay to fill it back up. Phil claimed to have capped the well with a plate before he buried it. There was one section of steel line and a gas regulator left at the site. |
1826944034 | 2018-06-06 | Final Restoration | No Violations | On the day of my inspection I found that the access road and well site were 80 to 90 percent covered in grasses and broadleaf vegetation. There were no signs of erosion and there were no signs that the plugged well was leaking. This site passes final restoration. |
