1169989711 2013-04-10 Orphan No Violations An inspection revealed that the well consisted of only an open 10 outside diameter drive pipe that was level with the ground surface. A depth counter equipped with a braided line and a 2 oz sinker was utilized for this inspection to gauge the fluid level and wellbore depth. The fluid level was measured at 24 and total depth was found at 43. The material encountered at 43 resembled a soft clay-like material. The fluid inside this casing did not exhibit gas bubbling, and no gas was smelled or detected at the surface with a gas meter. The well was situated on property designated as agricultural field, but surrounded by an approx 40 x 25 area of trees. The well location was at a slightly lower elevation and contained standing water in various spots to the west of the wellbore. There was no observed gas bubbling within the fluid or additional casing. Low Range Quantabs were placed within the fluid at three locations and all tests showed 0.8 units, a Chloride (Cl-) concentration below the US EPA recommended aesthetic value of 250 ppm (mg/L) of Cl-. Note: 1.2 units equals 29 ppm (mg/L) Cl-. The property also contained an old pit located below the well site that measured approx 50x25x5 with trees approx 6-8 diameter. A GPS reading was obtained and recorded for the access road/driveway entrance located at 21535 TR 379.

1452826383 2012-08-06 Orphan No Violations I conducted an inspection of the well site with ODNR supervisor Rocky King. I found that the well site was approximately 350 from the road, in a small patch of trees to the south of the driveway. The well consisted of open casing approximately 6 above ground level. I did not detect any gas odor or hissing coming from the open casing, nor did I detect any oil, brine or vegetation kill zone in the area. Tree removal will be needed to access the well for plugging operations.
