The crude oil stains were still sporadically present throughout the areas that were impacted, and the straw bails were still in place. I found no standing crude oil and nothing had collected in front of the straw bails. I called Dale Smith to ask what the plan was. He said that they would be out within the next two days to finish everything up. Dale said that he would call me the morning of the day they are going to be there. |

0605535651 | 2018-05-29 | Production Wells | No Violations | On the day of my inspection I met Dale Smith at the site to walk through the clean-up and address any remaining concerns. Dale said that they hauled approximately 60 tons of material to Kimbles landfill. The entire length of the roadside ditch had been cleaned out from the load line valve to the driveway culvert where the oil split towards the pond and hay field. The top layer of affected soil had been removed from the area in front of the pond, and in the hay field on the opposite side of the township road. There was a half circle of straw bales and absorbent booms set up in front of the pond, and the culvert. Dale and I found some small areas where oil staining was coming up through the soil. The oil stain that crossed the road was barely visible. Dale said that he would either have the backhoe come back out or he would get a crew out to hand shovel the remaining oil stains. I asked Dale to call me when they were done and I would come back out for a final walk through. |
0985696745 | 2018-05-24 | Production Wells | No Violations | On the day of my inspection I met Troy Cole and Rob McMasters with the Coshocton County EMA at the site. Dale Smith, the field superintendent for Diversified Oil and Gas, was leading a crew on the clean up. Vicky Bunting with Ohio Division of Oil and Gas Resources Management, Emergency Response Section also met me at the site. I found that the well was equipped with a natural gas powered pumping unit that was surrounded with livestock fence. The production equipment consisted of one 100 barrel steel tank in a shallow earthen dike and one 20 barrel poly tank on the fence line near the road. Both tanks were located within a fenced livestock area, and there was fencing around the dike. There was a steel load line that ran from the 100 barrel tank to the road for fluid pick up. The valve had been broken off the end of the line. The Diversified crew claimed that a cow got into the fence around the dike and knocked the valve open, releasing the fluid in the tank, which flowed through the open ended load line and into the roadside ditch. There was a little oil around the front of the tank, but there were no holes or damage to the front of the tank. The pumper claimed that when the cow allegedly knocked the valve open, it broke the seal and released some oil at the tank valve. The crew claimed that the tank was half full when the release occurred, estimating the total volume at 40 to 50 barrels. The oil followed the roadside ditch for approximately 600 feet north, where it split off and crossed the road, traveling northwest another 75 feet and migrating into a hay field. There was a heavy saturation of oil in the soil on the east side of the road where it split off. The saturated area was in front of a pond. The crew had dug a containment ditch to prevent the oil from reaching the pond. I walked the pond edge and found no oil had passed the ditch, and there was no oil or sheen on the pond water.
