0055739256 2016-06-30 Plug / Plug Back No Violations On the day of my inspection I found that Phil shut down after starting to pull the 5.5 casing. The casing was hung when he started to pull again. He made many attempts to pull the casing with no success. Phil came up and ripped the casing again at 945. He still couldnt get it to pull. He then made two more attempts at 882 and 760 before he could get the casing to come free. He pulled the 760 of free casing from the well. Then he started washing clay down the well to fill it from the brush and stone bridge at 1034. Phil washed approximately 12 more tons of clay down the well, filling it to surface multiple times. The clay would settle and they would fill it again. Once the well was filled and the clay was not falling, Phil pushed the remaining clay pile over the open casing and the well was left to sit over the weekend.

The well had been sitting open for 4 days. I told Phil that he was clear to cut the casing off and cap the well.

There is no identification (ID) posted at the well site The well owner must affix a valid and legible ID as soon as possible. This well must be put into production or plugged.
0984707975 2016-07-06 Plug / Plug Back No Violations On the day of my inspection Phil Moran uncovered the open conductor casing and the clay was still sitting firm at the surface. He cut the casing off 36 below grade and capped the casing with a steel plate inscribed with the permit number. The soil was pushed back over the well and the site was rough graded. This violation is resolved.

1467292339 2016-06-29 Plug / Plug Back No Violations On the day of my inspection Phil Moran washed 5 tons of clay down the 5.5 casing, then he rigged up to rip the 5.5 casing. He could not get any stretch when he tried to pull on the casing. He could not get a telegraph until 995. The 10 conductor was open with the 5.5 on a casing hanger. There were no other visible casing strings in the hole. The well was drilled in 1936. The 8 and 6 casing strings were likely removed during the WWII era and never recorded as being pulled. The 10 casing has likely been open for decades, allowing water, soil and debris to wash down the annulus around the 5.5 casing. Phil set a brush and stone bridge in the 5.5 casing at 1034 and then he ripped the 5.5 casing at 995 and started to pull it.
1762254860 2017-04-25 Final Restoration No Violations On the day of my inspection I found that the well site and access road were 90% covered in grass. There are two areas along the old access road that are always wet with standing water, so the grass did not grow there. I did not find any materials or debris left at the well site. There were not any tanks for this well. This site passes final restoration.
