0287145279 2018-05-30 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #6-1193. At the time of my inspection, I met ODNR-DOGRM surveyor Lucas Smith, who was on location to survey the well. Mr. Foltz arrived later with the modified fishing tool and was able to recover the tubing from the well. After removing the tubing, Mr. Foltz rigged up to determine a fluid level and began to bail fluid from the well bore. The fluid was contained in a lined earthen pit. Freeboard was maintained during bailing operations and I did not observe any leaks. Mr. Foltz reported that he planned to attempt to bail the well dry followed by a few days to observe recovery prior to plugging. I will schedule to be present during subsequent operations.
0426675325 2018-06-06 Plug / Plug Back No Violations The purpose of my inspection was to document the status of the surface plug in well #6-1193. At the time of my inspection, the plug remained at surface. I did not observe any fluid or gas leaks. The crew was rigging down and preparing to move to the next location.
0606585391 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with 8 casing, 6 1/4 casing, 2 tubing and rods. The pumpjack has been removed from the well. The annulus is open to the atmosphere. There is no tank battery associated with the well. There is no known production ever recorded for the well. The well owner is currently working to plug other berea wells that they own.
0736122900 2018-06-05 Plug / Plug Back No Violations The purpose of my inspection was to witness the attempt to remove casing and setting of the surface plug. The top of cement was tagged at 370 feet below surface. After making several attempts to pull the 6.25 surface casing string above 20% of string weight, owner Bob Foltz pulled over weight on the casing and backed the casing out. A total of 154 of surface casing was recovered. Mr. Foltz installed 2 tubing back in the well to a depth of 338 feet. R.W. Sidley delivered and poured 3.75 yards of 9 sack grout down the backside of the tubing. Flow was established from the tubing and the grout was brought to surface. Mr. Foltz removed the tubing from the well and the displacement was topped off back to surface. The grout had surface held. The location remained stable. I will schedule a follow-up inspection to document the status of the surface plug prior to backfill.
1312764859 2018-06-04 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the bottom plug in well #6-1193. At the time of inspection, I did not witness any flow from the well. Owner Bob Foltz was unable to dry the well bore. Mr. Foltz installed tubing into the well bore to act as a tremie line. R.W. Sidley delivered 2.5 yards of 9 sack grout. The grout was poured into the well bore and flow was observed from the tremie line. Once flow stopped from the tremie line, Mr. Foltz removed the tubing from the well. Mr. Foltz picked up a set of jars and ran into the open well bore and determined the top of cement to be around 462. I will schedule to be present to witness the attempt to pull casing and setting of the surface plug.
1527793398 2018-05-29 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #6-1193. Owner Bob Foltz reported that upon disassembling the well head, he discovered the tubing at parted at surface. At the time of my inspection, Mr. Foltz was rigged up with fishing tools to recover the tubing. No flow was observed from the well. Mr. Foltz made several attempts to recover the tubing but was unsuccessful. He reported that the plan was to modify the tool used to recover the tubing and make a second attempt on 5/30/2018. I will be schedule to be present to witness additional operations.
1547557709 2019-01-15 Final Restoration No Violations The purpose of my inspection was to document the final restoration status of well #6-1193. The well location was within an agricultural field. At the time of my inspection the location was partially covered in snow. The pit was backfilled. No equipment remained. No buried flow lines existed. No debris was present. No production equipment was on location. The location was stable. I did not witness any signs of erosion. No access road was present. The location meets the requirements for final restoration.
