0090227727 2017-04-14 Production Wells No Violations The purpose of my inspection was to document the progress of plugging well #6-1115. The well owner had his service rig on location along with a farm tractor and trailer. Mr. Watson had not removed the tubing head or began to remove rods and tubing at the time of inspection. I will continue to communicate with Mr. Watson regarding operation schedules in order to be present during plugging activities.
0129067763 2017-04-24 Plug / Plug Back No Violations The purpose of my inspection was to witness the removal of rods and tubing from well #6-1115. I arrived on location with ODNR-DOGRM Northeast Regional Manager Thomas Hill and found the twenty-four (24) rods removed from the well, each roughly twenty-six (26) feet long. Mr. Watson had also removed seventeen (17) joints of tubing that roughly tallied 313. Following Mr. Hills departure, Mr. Watson arrived on location to continue work. Mr. Watson stated that the bottom valve in the working barrel was stuck and holding fluid in the tubing. He planned to run a retrieval tool into the tubing to attempt to recover the bottom valve and drain the fluid into the well bore. Mr. Watson ran the tool on rods to the bottom valve and attempted to screw into the assembly. I witnessed oil being displaced as rods were tripped into the tubing. Once on bottom, Mr. Watson attempted to thread the downhole tool into the bottom valve assembly. The working barrel unthreaded itself off of the last joint of tubing. Mr. Watson removed the rods and downhole tool from the tubing and the bottom valve was not attached. I inspected the threads of the last joint of tubing removed from the well and found the threads in fine condition, with no evidence of damage. The tubing was free of fluid following the fishing run. Mr. Watson state he planned to move onto well #6-1114 and remove rods and tubing from that well next and planned to plug well #6-1114 first. I will continue to coordinate schedules with Mr. Watson in order to be present during plug and abandon operations.
0191147376 2015-03-31 Production Wells No Violations I was onsite to conduct a status check with Inspector Kevin Perry. The wellhead has been converted from a shackle line run pumpjack to a gas engine pumpjack, but the gas engine is missing and the shackles have been removed. The pumpjack has since been removed completely. The well consists of 6.25 casing, 2.375 tubing, and rods. No identification was posted. Well not capable of production.
0213893051 2017-05-31 Plug / Plug Back No Violations The purpose of my inspection was to witness the placing of the bottom producing zone isolation plug in well #6-1115. I received a message from Owner Shawn Watson that he planned to measure the total depth of the well after bailing all fluid from the well bore. I winessed Mr. Watson measure the total depth at 650 feet below surface. Original total depth was reported as 712 feet. The top of the Berea formation was reported at 654 feet. I spoke with ODNR-DOGRM Northeast District Supervisor concerning the fill up. Supervisor Ochs gave permission to proceed setting the bottom isolation plug. Mr. Watson stated he made a final bailer run at 14:20 and recovered approximately 2 gallons of mostly salt water. The previous bailer run was 11:45. R.W. Sidley delivered 1.25 yards of 9 sack grout at 14:50. The plug was set at 15:45. I will schedule to witness Mr. Watson measure the top of the cement plug.
0321620361 2017-05-19 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the top plug in well #6-1115. A total of 335 of 5 casing was removed from the well. A measuring line was used to tag the top of fill at 447 below surface after removing the casing. Tubing removed from the well was run to a depth of approximately 428 below surface to as a tremie pipe. RW Sidley Cement delivered 4.0 yards of 9 sack grout. The grout was mixed to an approximate 5 inch slump and poured down the tubing annulus. The tremie tube did not circulate while grout was being poured. The tubing was removed from the well bore and the fluid level tagged at 10 feet below surface. I will confirm the top of cement prior to Mr. Watson cutting the casing below grade and welding on the identification plate.
0435288962 2017-05-17 Plug / Plug Back No Violations The purpose of my inspection was to witness well conditions and the setting of the bottom plug in well #6-1115. Mahoning County Inspector John Fleming was present during the grouting process. After 24 hours, Mr. Watson had baled approximately 25 gallons of fluid from the well. I witnessed Mr. Watson continue to bale the well while waiting for the grout truck. Approximately one gallon of fluid was being recovered during each baler run. The fluid at this time contained little to no oil and was comprised mainly of gray colored water. Sidley Cement arrived on location with 1.5 yards of 9 sack grout. The grout was mixed to a semi-thick consistency and slowly poured into the well bore to avoid bridging. I will meet Mr. Watson on location on 5/18/2017 to witness measurement of the top of cement prior to his attempt to pull the 5 casing from the well.
0619196193 2017-06-07 Plug / Plug Back No Violations The purpose of my inspection was to document the status of the surface plug set on 06/06/2017. The 9 sack grout poured into the well bore remained at surface. Owner Shawn Watson was working on rigging down the location and removing remaining equipment. Mr. Watson stated he had an operator scheduled to excavate the well head area and to weld on the memorial plate. No violations were observed during this inspection.

1092887762 2017-06-20 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #6-1115. The excavated well head area had been graded back to local contour and cultivated by the surface owner. No pits were utilized in plugging operations. No well equipment utilized or recovered in plugging operations was on location. The tank battery remained on location to service additional wells on the lease that will be plugged at a future date. Flow lines from the well to the tank battery were not removed. I will contact owner Shawn Watson to submit a landowner waiver before passing final restoration.
1493058683 2017-04-24 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #6-1115. Northeast Region Supervisor Thomas Hill was present during the inspection. Mr. Watson was not present on location upon arrival. Owner Shawn Watson was rigged up on location and had pulled 24 rods from the well and 17 joints of tubing. There was a small excavation next to the well head utilized to collect crude oil that was trapped inside the tubing. After Mr. Hill left location, the surface owner, Mr. Robert Herold, arrived on location. We discussed the plugging operation and time frame needed to agricultural field. At 12:30, Mr. Watson arrived with a downhole tool designed to recover the bottom of the pump assembly. Mr. Watson explained that the pump was preventing oil inside the tubing from draining and he was going to attempt to screw into the top of the pump and pull the assembly out to allow oil to drain to the well bore. I witnessed Mr. Watson run 12 rods back into the well at which point he encountered a tight spot. Oil was being displaced and collected during this process. After rotating the rods through the tight spot, Mr. Watson added one additional rod and attempted to screw into the top of the pump assembly. I did not witness oil displacement on the last rod. Mr. Watson proceeded to trip out the rods. The bottom pump was not attached to the recovery tool. Mr. Watson explained that a packed off hole in the tubing may have broke and caused the oil to drain out. Mr. Watson proceeded to pull out 16 additional joints of tubing for a total of 33. The bottom pump barrel was not attached to the last joint. I inspected the threads of both the downhole recovery tool and last joint of tubing and both were in good repair. Mr. Watson stated to me that he planned to rig down and move to the Teegarden #12 (#6-1114) and remove the rods and tubing from that well before proceeding with plugging well #6-1114. I will plan to be present during the rod and tubing removal on well #6
1495798745 2017-05-24 Plug / Plug Back No Violations The purpose of my inspection was to document the plug and abandonment status of well #6-1115. I received a phone call from Shawn Watson on the evening of 5/23/2017 regarding a downhole obstruction he encountered while attempting to bail the well dry. The obstruction was encountered at 434, the same depth as the 4.875 casing seat. After discussion, the decision was made to first place tubing into the well bore in an attempt to remove the obstruction. I arrived on location to find Mr. Watson tripping tubing into the well bore. I witnessed Mr. Watson encounter the obstruction with the tubing. The weight of the tubing was enough to move the obstruction downhole. Mr. Watson planned to continue tripping tubing into the well bore in order to drive the obstruction to bottom. I received a follow up phone call later from Mr. Watson who stated he was able to push the obstruction to bottom and would resume bailing fluid from the well prior to setting the bottom zonal isolation plug. I will continue to maintain communication with Mr. Watson in order to be present during plugging operations.
1497025814 2017-06-08 Plug / Plug Back No Violations The purpose of my inspection was to document the plugging status of well #6-1115. The area around the well had been excavated and the well casing cut below the 30 inch minimum. A memorial plate identifying the permit number and date was welded to the casing. Surface owner Bob Herold was scheduled to back fill the location in preparing the agricultural field for planting. I will plan to document the final restoration status of the location when the location meets the criteria. Mr. Watson had removed all well equipment and casing from the location. No violations were observed during this inspection.

1948951736 2017-06-01 Plug / Plug Back No Violations The purpose of my inspection was to witness the measurement of the top of cement plug set the previous day. Well owner Shawn Watson marked the braided wire line on his service rig in 70 feet increments. A 30 feet long, 3 inch diameter bailer was attached at the end of the line and run in the hole to tag the cement top. The top of cement was calculated at 400 feet below surface. I observed a witness mark from cement on the bailer at 4 feet above the bottom. Mr. Watson emptied the bailer into a barrel on location and recovered approximately 1/2 gallon of fluid. Mr. Watson stated he planned to return the following week to attempt to pull the casing. I will schedule to be on location to witness the attempt to pull casing.
1968552270 2017-03-30 Production Wells Violations Noted The purpose of my inspection was to witness a field meeting concerning the status of well #6-1115 and discuss plans to plug and abandon the well. A meeting was held at the farm of surface property owner Mr. Robert Herold. Meeting attendees included the surface owner, Mr. Robert Herold, well owner, Mr. Shawn Watson, ODNR-DOGRM Northeast Regional Supervisor Thomas Hill and Enforcement Coordinator Richard Anderson. Mr. Watson stated he planned to apply for a plugging permit and would begin to prepare the well for plugging by removing tubing and rods from the well as soon as his service rig was repaired with a part that was ordered and field conditions were suitable for mobilizing the rig to the well head. Mr. Herold stated that he would schedule cultivation activities around plugging operations. Mr. Watson continued that the repairs should be completed within a week. Plans were discussed and tentatively approved to plug the well with 9 sack grout instead of clay. Mr. Hill communicated to Mr. Watson that he had thirty (30) days to obtain a permit to plug the well and sixty (60) days to have the work completed or face further enforcement action. I will follow up with Mr. Watson within a week to confirm that rig repairs have been made and determine the schedule to begin plugging the well.
2014561499 2017-06-06 Plug / Plug Back No Violations The purpose of my inspection was to witness the pouring of the surface plug in well #6-1115. The top of the open hole section of plug was tagged at 403. Per reported well construction records, the grout was filled up within the 4.875 casing. Shawn Watson attempted to pull the casing and was not able to pull the casing. After failing to pull the casing, Mr. Watson unscrewed the 4.875 casing and recovered 91. The condition of casing prevent Mr. Watson from attempting to attempt to recover additional casing. I called and spoke with ODNR-DOGRM Deputy Chief Robert Worstall concerning the situation. After explaining the plugging operations, Mr. Worstall agreed to allow plugging operations to proceed. Fluid level remained at 25 feet below surface. Mr. Watson inserted 380 feet of 2 tubing into the well to displace the fluid with grout. At 12:50, R.W. Sidley delivered 2.5 yards of 9 sack grout. Well bore calculations indicated 2.3 yards would fill the top section. I scaled the grout at 18.6 ppg. 2 yards of grout was poured into the well bore prior to pulling approximately 100 feet of tubing. During the pouring of the grout, fresh water was being displaced through the tubing into barrels Mr. Watson had on location. After removing 100 feet of tubing, grout was poured into the well and brought to surface. The remaining tubing was pulled from the well bore and remaining grout poured into the well up to surface level at 15:20. I will schedule to witness the cement top on 06/07/2017. No violations were observed during this inspection.
