0154145624 2017-05-18 Plug / Plug Back No Violations The purpose of my inspection was to witness well owner Shawn Watson attempt to pull the 5 surface casing from well #6-1114. Mahoning County Inspector John Fleming was also on location. Mr. Watson had two (2) 15 ton lever jacks placed under the well head. A steel bar was placed through the annular side ports of the well head for the jacks to push against. A chain was also utilized around the bar and attached to the service rig elevators to pull against the casing. Mr. Watson made several attempts to pull the casing before detaching the well head from the casing. Mr. Watson then cut two windows into the side of the casing to reposition the steel bar and reattempt to pull the casing. This attempt to pull the casing was unsuccessful. Mr. Watson then unscrewed the casing. After unscrewing and pulling up on the casing, trapped annular gas and fluid u-tubed into the well casing. After stabilizing, the fluid level was tagged at 45 feet below surface. Mr. Watson stated he would return on 5/19/2017 to remove the casing and prepare to set the top lug with 9 sack grout.
0233986379 2017-03-30 Production Wells Violations Noted The purpose of my inspection was to witness a field meeting concerning the status of well #6-1114 and discuss plans to plug and abandon the well. A meeting was held at the farm of surface property owner Mr. Robert Herold. Meeting attendees included the surface owner, Mr. Robert Herold, well owner, Mr. Shawn Watson, ODNR-DOGRM Northeast Regional Supervisor Thomas Hill and Enforcement Coordinator Richard Anderson. Mr. Watson stated he planned to apply for a plugging permit and would begin to prepare the well for plugging by removing tubing and rods from the well as soon as his service rig was repaired with a part that was ordered and field conditions were suitable for mobilizing the rig to the well head. Mr. Herold stated that he would schedule cultivation activities around plugging operations. Mr. Watson continued that the repairs should be completed within a week. Plans were discussed and tentatively approved to plug the well with 9 sack grout instead of clay. Mr. Hill communicated to Mr. Watson that he had thirty (30) days to obtain a permit to plug the well and sixty (60) days to have the work completed or face further enforcement action. I will follow up with Mr. Watson within a week to confirm that rig repairs have been made and determine the schedule to begin plugging the well.
0321620361 2017-05-19 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the top plug in well #6-1114. A total of 335 of 5 casing was removed from the well. A measuring line was used to tag the top of fill at 447 below surface after removing the casing. Tubing removed from the well was run to a depth of approximately 428 below surface to as a tremie pipe. RW Sidley Cement delivered 4.0 yards of 9 sack grout. The grout was mixed to an approximate 5 inch slump and poured down the tubing annulus. The tremie tube did not circulate while grout was being poured. The tubing was removed from the well bore and the fluid level tagged at 10 feet below surface. I will confirm the top of cement prior to Mr. Watson cutting the casing below grade and welding on the identification plate.
0770779070 2017-05-31 Plug / Plug Back No Violations The purpose of my inspection was to follow-up on compliance notice # 233986379. At the time of my inspection, owner Shawn Watson had topped off the top of the surface cement plug with two bags of sakrete mix to bring the cement top to surface. The previous measure top of cement was 10 feet below surface. The casing was cutoff 36 below surface and I witnessed Mr. Watson weld on a monument plate with correct information. Mr. Watson stated that the surface owner (Mr. Robert Herold) will back fill the well head area prior to planting the field in crop. I will schedule a follow-up inspection to determine the status of final restoration. No further action is necessary at this time.
1003577227 2015-03-31 Production Wells No Violations I was onsite to conduct a status check with Inspector Kevin Perry. The wellhead has been converted from a shackle line run pumpjack to a gas engine pumpjack, but the gas engine is missing and the shackles have been removed. The pumpjack has since been removed completely. The well consists of 6.25 casing, 2.375 tubing, and rods. No identification was posted. Well not capable of production.

1693868981 2017-06-20 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #6-1114. The excavated well head area had been graded back to local contour and cultivated by the surface owner. No pits were utilized in plugging operations. No well equipment utilized or recovered in plugging operations was on location. The tank battery remained on location to service additional wells on the lease that will be plugged at a future date. Flow lines from the well to the tank battery were not removed. I will contact owner Shawn Watson to submit a landowner waiver before passing final restoration.
1970091568 2017-05-15 Plug / Plug Back No Violations The purpose of my inspection was to witness the bailing of well #6-1114. I received a phone call from Shawn Watson stating that he had bailed the well dry and was monitoring the well to determine the rate of production. Mr. Watson stated he had bailed the well dry the previous night and as of that morning, had bailed approximately 20 gallons of fluid. I witness one final bailer run for the day and observed approximately one gallon of fluid drain from the bailer. Mr. Watson stated he planned to have Sidley deliver 1.5 yards of 9 sack grout on 5/16/2017 to set the bottom plug. No log was available and Mr. Watson was unable to determine the depth of the casing shoe. Calculations are based on well #6-1115 450 feet to the East. No violations were observed during this inspection.
2007923256 2017-05-04 Plug / Plug Back No Violations The purpose of my inspection was to document the status of plugging well #6-1114. I received a phone call from Mr. Shawn Watson on 4/28/2017 reporting that the during his attempt to remove tubing from the well, the tubing parted at the surface and fell to bottom. He stated that he was looking for a tubing spear to run in the hole to recover the tubing and continue to trip the tubing out of the hole. On 5/3/2017, I receive a text message from Mr. Watson stating that he had successfully removed the tubing from the hole. I arrived on location the morning of 5/4/2017 to find the rods and tubing removed from the well. I counted twenty-eight (28) rods and thirty-five (35) joints of tubing on location. The location was well-kept and organized. There was no stray production fluid on the ground. I spoke with Mr. Watson and he stated that once field conditions dry out from recent precipitation, he plans to move his swab rig over the well and proceed with plugging operations. No violations were observed at this time.
