0781697194 2005-03-15 Production Wells Violations Noted There has been no remedial action taken. Requested a Chiefs Order 03/17/2005. The company has not obtained surety. The well remains idle. The well continues to leak oil. The contamination around the wellhead and tank battery has not been removed. There is no identification posted. There is no dike around the tank battery.
0809544395 2005-01-12 Production Wells Violations Noted The well is idle. There is no bond, no insurance, no dike, no identification and no bonded party found to take over the wells. There is wellhead and tank battery contamination.
1105042367 2004-05-12 Production Wells No Violations A meeting between Tom Tugend, Tom Hines, Wayne Schalk, Jonnie Patrick and Randy Brown took place to discuss the status of the Berea wells on the Brown property. Mr. Brown claims to own the equipment but not the holes. It was agreed that Mr. Brown and the Division would try to find a bonded party to assume ownership to plug or produce the wells. Obtained GPS coordinates. The tank battery is shared with multiple wells and consists of 2-100 bbl, 1-30 bbl and 1-unknown size storage tanks and no dike. There is contamination around the tanks and to the north and west along the fence row. The size is approximately 100 by 10. There is contamination around the wellhead and to the north approximately 30 by 50. There is no bond, no insurance, no identification and no production statements filed. The well is idle. No enforcement action is to be taken at this time.

1570734598 2015-06-22 Production Wells No Violations My inspection found the well to be equipped with 8 conductor casing, 6.25 production casing, 2.375 tubing and rods. The gas engine powered pump jack has been removed from the well and is sitting next to it. There is a service rig that broke down while the rods were being removed from the well. The well is actively leaking oil and brine from the packing seals between the rods and the tubing. The well is also leaking from a disassembled valve body that is on the flow line. The area around the well that is 10 ft. by 20 ft. is saturated with oil and brine. The tank battery consists of two 200 BBL production tanks, a 75 BBL tank, a 50 BBL stainless steel tank, and two mechanical pumps. There is a brine scald on the North West side of the tank battery that is 10 ft. by 100 ft. and oil saturated ground that is 10 ft. by 20 ft. on the East side of the tank battery. The well is not in production and there is no record of production reported for the well. The tank battery is being shared by 13 Berea wells on the property. There is no identification on the well or the tank battery. The well is easily accessed by a lease road and a open field.
